Clinical study on application of a multifocal apodized diffractive intraocular lens(Acrysof ReSTOR)Purpose: to evaluate the safety and effectiveness of the AcrySof ReSTOR apodized diffractive intraocular lens (IOL), model MA60D3, when implanted into the capsular bag,and to compare the visual outcome and subjective visual symptoms in patients who had implantation of the AcrySof ReSTOR multifocal intraocular lens (IOL) with those in patients who received the AcrySof SA60AT monofocal IOL in cataract surgery.Methods:thirty-four eyes of 32 patients received the ReSTOR multifocal IOL after cataract extraction. They were closely paired in age, sex, and ocular findings with patients who had monofocal IOL implantation during the same period. The distance and near visual acuities ,pseudoaccommodation,contrast sensitivity with and without glare were compared, and patient satisfaction with their vision, subjective visual symptoms, and spectacle dependence was assessed by using a standardized questionnaire. All parameters were evaluated 3 months after surgery.RESULTS: At 3 months postoperatively,in MuIOL group:UCDVA was 4.93±0.019,BCDVA was 4.987±0.156,UCNV was 0.66±0.23,BCNVA was 0.75±0.20 and depth of focus was 6.0D,the pseudoaccommodation was +2.73±0.47D;while in MoIOL group:UCDVA was 4.92±0.02,BCDVA was 4.96±0.168 ,UCNV was 0.17±0.10,BCNVA was 0.73±0.20 and depth of focus was 4.0D,the pseudoaccommodation was +1.37±0.46D. An uncorrected distance visual acuity of 4.8 or better was achieved in 91.17% eyes in the multifocal group and 91.17% in the monofocal group. Uncorrected near visual acuity was 0.5 or better in 85.30% of eyes in the multifocal group and 5.58% in the monofocal group.There was no significant difference in the mean uncorrected and best corrected distance visual acuities between the groups. There was significant difference in the mean uncorrected near visual acuities and pseudoaccommodation between the groups.The multifocal group had significantly better near uncorrected acuity.Contrast sensitivity was similar between the groups.Reading glasses were required by 8.82% in the multifocal group and by 70.59% in the monofocal group.Satisfactory uncorrected intermediate distance vision was achieved in 88.2% of eyes in the multifocal group and 91.1% in the monofocal group.Moderate glare was reported by 17.65% in the multifocal group and 5.88% in the monofocal group.CONCLUSION: The AcrySof ReSTOR IOL with its apodized diffractive optic provides patients with excellent near visual acuity without compromising distancevision ,signifi-cantly reducing of severe visual disturbances compared with monofocal controls.
