Object:The purpose of the study is to examine the expressive quantity of cell surface receptor on Monocaryon / Macrophage;furthermore investigate the mechanism related spontaneous rupture of hepatocellular carcinoma.Method:In this study,we used to look-forward experimentation of randomized control trial to collect each 9 specimens from spontaneous rupture of hepatoceUular carcinoma and non-rupture hepatocellular carcinoma.Preoperatively,peripheral blood of spontaneous rupture of hepatocellular carcinoma is drew-off.In the operation,tumor tissue specimens of the same patient is choosed.The method of choosing specimens in the patients of non-rupture hepatocellular carcinoma is the same.The specimens are used to examine: Quantification investigate to the expression of cell surface receptor of CD11b,CD16, CD18,CD32 on Monocaryon and on heoatic macrophage in FCM in order to analysis the dependability of cell surface receptor of CD11b,CD16,CD18,CD32 on monocaryon / macrophage and ability of macrophage to swallow IC;qualitative analysis to locations and distributions of hepatic macrophage in immunohistochemical analysis.Results:Quantificationally investigate to expressions of CD11b,CD16, CD18,CD32 on monocaryon/macrophage in FCM from cases with spontaneous rupture of hepatocellular carcinoma and non-rupture hepatocellular carcinoma:①Expression percents of CD11b,CD16,CD18,CD32 on peripheral blood monouclear cells from spontaneous rupture of hepatocellular carcinoma are lesser than the cases from non-rupture hepatocellular carcinoma,with statistical significance[12.73(11.19, 13.98) vs 17.91(16.11,19.21),Z=-1.911,P=0.001];[8.33(7.07,8.54) vs 12.23(10.61, 12.99),Z=-1.876,P=0.026];[9.10(8.04,10.42) vs 14.93(12.47,16.01),Z=-2.296, P=0.020];[9.26(7.62,10.03) vs 10.79(9.79,11.37),Z=-1.997,P=0.045];②Expression percents of CD11b,CD16,CD18,CD32 on hepatic macrophage from liver carcinoma tissues of spontaneous rupture of hepatocellular carcinoma are lesser than the cases from non-rupture hepatocellular carcinoma,with statistical significance, [8.46(7.52,9.02) vs 11.66(9.55,12.05),Z=-1.993,P=0.076];[2.79(2.27,2.94) vs 3.85(3.03,5.02),Z=-3.151,P=0.039];[6.03(5.10,6.27) vs 9.48(8.18,9.72), Z=-2.353,P=0.001];[3.22(2.23,3.72) vs 6.56(5.09,7.67),Z=-1.291,P=0.001]③Expression percents of CD11b,CD16,CD18,CD32 on hepatic macrophage from non-carcerous liver tissues of spontaneous rupture of hepatocellular carcinoma are lesser than the cases from non-rupture hepatocellular carcinoma,with statistical significance,[10.79(9.63,3.38) vs 15.88(14.12,17.13),Z=-1.616,P=0.001];[5.44 (4.87,5.84) vs 11.19(9.64,11.93),Z=-3.624,P=0.001];[6.03(5.10,6.27) vs 13.90(11.50,14.42),Z=-2.205,P=0.018];[5.73(5.11,6.60) vs 9.70(8.57,10.20), Z=-1.291,P=0.085].④Expression percents of CD11b,CD16,CD18,CD32 on monocaryon/ macrophage of spontaneous rupture of hepatocellular carcinoma are lesser than the cases of non-rupture hepatocellular carcinoma and that have noting to do with the age,gender,AFP,HBsAg,cirrhosis and the size of tumor(P>0.05).⑤Expression percents of CD11b,CD16,CD18,CD32 on monocaryon/macrophage of spontaneous rupture of hepatocellular carcinoma show positive correlation of straight line(0<r<1,P<0.05).⑥Results of immunohistochemical analysis showed all presences of macrophage in cases with rupture HCC and non-rupture HCC.They appeared spindly or star-shaped,their cytoplasm appeared brown-yellow,located venous cavities in liver carcinoma tissues and adjacent non-cancerous liver tissues.Conclusion:Phagocytosis has been damaged before histomonocyte becomes macrophage in the liver and we can conclude the dysfunction of macrophage,which could result in the cumulating ang deposition of IC and vascular injury,may be the factors involved in the pathogenesis of spontaneous rupture of hepatocellular carcinoma.
