The Significance Of Radical Radiotherapy For Regional Recurrence In Esophageal Carcinoma After Radical Surgery And Its Prognostic Fators

Objective: To study the significance of radical radiotherapy for regional recurrence in esophageal carcinoma after radical surgery and its prognostic factors. Methods: From January 2000 to January2005 , 91 patients who developed regional recurrence in esophageal carcinoma after radical resection were analyzed retrospectively :in which 20cases are recrudescence, in the supra-clavicle,34 cases in the mediastinum,23cases in mediastinum plus supra-clavivale, 4 cases in anastomoses-orifice,6cases in mediastinum plus anastomoses-orifice,and4 cases in abdominal lymph node.Radiotherapy was delivered by conventional fractionation.A total dose of 50Gy～70Gy was given.68 cases received chemotherapy after radiotherapy. Results:The recrudescence-time was1~35 months,The overall median recrudescence-time was 11.1months . The survival time was 2.8~45 months. The overall median survival time was 11 months and 1 ,2 , 3 year survival rates were 51.7%,20.4%,14.4%, respectively. By univariate analysis ,it was proved that recrudescence-time, postoperation surgical stage ,dose of radiaotion ,response rates ,T stage and N stage were related with prognosis . By multivariate analysis,it was proved that recrudescence-time postoperation surgical stage ,dose of radiaotion and response rates were independent prognostic faters for the survival. Conclusion: Radiotherapy may improve the survival in patients with postoperative recurrence in esophageal carcinoma. Patients with recrudescence-time long ,early lesion found at surgery and highly response rates ,dose of radiaotion higher than 60Gy have better prognosis .
