Objective:To explore the therapeutic effects of partial splenicarterial embolization(PSE)combineing with transhepatic arterial infusion of salvia miltiorrhiza injection in treating hypersplenism caused by hepatocirrhosis.Materials and Methods:68 cases suffered from hypersplenism caused by hepatocirrhosis in accordance with the inclusive criteria from Sept.2005 to Mar.2008 were divided into two groups.37 patients in the treatment group were treated by PSE combining with transhepatic arterial infusion of salvia miltiorrhiza injection.31 patients in the control group were treated by simplicity PSE.All patients were observed white blood count(WBC),red blood count(RBC),platelet count(PLT),liver function,hepatic fibrosis indexes, hemodynamic indexs of portal vein(PV)and splenic vein(SV)before and after PSE.And observed reactions and complication after PSE.Results:As compared with preoperation,the WBC and PLT rosed significantly after one week,and gradually decreased and stabilized at the normal level one month later,but RBC did not change significantly.ALT,AST,ALP,γ-GT and TB of two groups rosed significantly after one week(P<0.05),but A/G,TP and PT did not change significantly.ALT,AST,ALP,γ-GT,TB,A/G,TP and PT decreased significantly one month later after PSE(P<0.01).but the decreased degree in control group was milder than in treatment group(P<0.05).The hepatic fibrosis indexes HA,PHIP and LN decreased significantly one month later after PSE(P<0.01),and the decreased degree in control group was milder than in treatment group(P<0.05)too.One day after PSE the diameter of PV and SV reduced,the blood flow velocity increased and the blood flow volume decreased.But the blood flow volume of PV decreased after one month, while which of SV did not decreased again.The esophageal varices were ameliorated after PSE,the improvement rate of treatment group was 34.78%. It was better than the control group which had a improvement rate of 20.00%. The Child-Pugh score decreased in different degree after PSE.The average time of fervescence was 9 days.The average time of persistent pain was 5 days. No severe complication occurred in all cases after operation.The recent efficacy was satisfactory.Conclusions1:1.It is indicated that PSE can significantly remit hypersplenism,decrease portal pressure and improve liver function.2.The treatment of PSE combining with transhepatic arterial infusion of salvia miltiorrhiza injection improves liver function and hepatic fibrosis indexes effectively.This method can be finished in the single course,and it is simple and low price.Therefore it can be used as an important adjuvant therapy or one operation procedure of PSE.
