BackgroundLaparoscopic surgery has been widely considered nowadays due to the advantages of less invisive, more rapid recovery and shorter hospital stay . But the injury of operation often causes hemodynamic changes. Flurbiprofen Axetil is used clinically for postoperative pain relieve. Some research shows that prostaglandin D2 and E2 can modulate the of sleep-wakefulness cycle, may affect the depth of sedation. Nonsteroidal anti-inflammatory drug (NSAIDs) can control the postoperative pain by reducing prostaglandin production through inhibiting cyclooxygenase. Flurbiprofen is one kind of NSAIDs, it reduces the production of prostaglandin and selectively accumulates in the inflammatory tissue and injured site. Thereby Flurbiprofen Axetil may influence the depth of sedation and hemodynamics during surgery.ObjectiveThis research aims to find whether administration of Flurbiprofen Axetil before elective laparoscopic cholecystectomy can affect the BIS index and hemodynamics.MethodSixty patients scheduled for elective laparoscopic cholecystectomy were randomized into two groups. Flurbiprofen axetil 1mg/kg was used intravenously 15 minutes before induction of general anesthesia in group B, while group A without premedication. Total intravenous anesthesia maintained with propofol TCI. BIS index, bloodpressure and heartrate were recorded at the timejust before induction, after endotracheal intubation, 5 minutes before incision, incision, 5, 10 , 15, 20 and 25 min after incision.ResultThere is no significant difference between two groups in BIS index during period of anesthesia and awakeness (all p>0.05). Compare with group A, both systolic and diastolic blood pressure in group B at time of incision(T1) are apparent lower(p<0.05). While, blood pressures were no significant different at other time (all p>0.05).ConclusionIn this study, administration of flurbiprofen before surgery of laparoscopic cholecystectomy did not alter BIS and the degree of sedation during total intravenous anesthesia. But it can reduce the harmful stimulation to cardiovascular reaction from surgery and make hemodynamic status more stable. Flurbiprofen can be safely and effectively used on elective laparoscopic cholecystectomy.
