Objective:To study the prognostic factors of patients with endometrial carcinoma. To compare the clinicopathologic character and prognostic factors of endometrial carcinoma in premenopausal and postmenopausal women.Methods:154 patients with endometrial carcinoma were accepted by primary surgical treatment from Jan.1995 to Dec.2003 at the gynecology dept of Guangxi Tumor Hospital.Only 139 patients who had enough information were assessed retrospectively.We divided the patients into two groups according to the menopausal state.54 cases were premenopausal women and 85 cases were postmenopausal women.Then we studied clinicopathologic character and prognostic factors of the two groups.Life Table method was used to evaluate the survival rates,the Univariate analysis was estimated by Log-rank test,the multiple factors were assessed by Cox regression and x~2 test was used to compare clinical pathological variables according to the menopausal state.Results:The total 1,3,5,8 years survival was 90%,78%,69%and57%,respectively, univariate model revealed that the menopausal state,the FIGO stage,grade, histological type,myometrial invasion,ER,PR and the method of operation associated with the prognosis.The multivariate analysis showed that the FIGO stage,grade,histological type,myometrial invasion,ER and PR were related with the prognosis significantly.When the premenopausal group was compared with the postmenopausaul group,the histology grade was lower(P=0.035),the deep myometrial invasion was less(P=0.023)with more ER(P=0.016),and higher survival rate(P=0.010).The differences were significant in all the aspects mentioned above(P<0.05).Conclusion:The prognosis of endometrial carcinoma were associated with high risk factors.Early diagnosis and reasonable treatment can improve the prognosis. The clinical and pathologic characteristics of endometrial carcinoma might be responsible for the better prognosis of premenopausal patients compared with postmenopausal ones.In order to improved the prognosis of the patients in endometrial carcinoma,they should be treated by individual therapy,...
