Objective To study the value of real-time three-dimensional echocardiography(RT-3DE)in evaluate the left ventricular remodeling after myocardial infarction.Methods Thirty patients with LVR and thirty healthy volunteers were evaluated LVEDV,LVESV,LVEF,LVM,LVRI by MME,2DE,and RT-3DE. The LVR patients calculated Gensini score based on the CAG outcome.We had compared the difference of each value between LVR patients and healthy volunteers,and evaluated the correlations between the value and the Gensini score,which were obtained by MME,2DE and RT-3DE respectively.Result(1)In healthy volunteers,there was no significant difference among the value obtained by MME,2DE and RT-3DE(P>0.05).(2)But there was significantly difference in LVR patients among the value obtained from RT-3DE and MME,2DE.(P<0.05).(3)There was significantly difference between the value of LVR patients and healthy volunteers,which was measured by three methods respectively.The LVEDV,LVESV,LVM in LVR patients were greater than those in the healthy volunteers(P<0.05),while the LVEF,LVRI were less than them(P<0.05),which implied left ventricular had been enlarged and the systolic function had been degraded in LVR patients.(4)There were well correlation between LVEDV,LVESV,LVEF,LVRI measured by RT-3DE and Gensini score(r=0.844,0.880,—0.839,—0.854),and the correlations were better than MME,2DE.But there wasn't significant correlation between LVM and Gensini score(r=0.431).(5)RT-3DE could show the whole shape of LV, and show the irregularity change of left ventricular shape after myocardial infarction graphically and directly.Conclusion(1)RT-3DE can accurately measure left ventricular value.The LVRI can quantitatively evaluate LVR.(2)RT-3DE could show the irregularity change of left ventricular shape in patients with left ventricular remodeling after myocardial infarction,graphically and directly.(3)There had increased in LVM early after the myocardial infarction left ventricular remodeling,but the dilatation of left ventricular was predominant.There was positive correlation between the LVRI and the degree of coronary artery stenosis.The characteristic of old myocardial infarction and the LVR progress need further study.
