Expression Of Epidermal Growth Factor Receptor (EGFR) P-glycoprotein (P170), Topoisomerase Ⅱ (ToPo Ⅱ) And Glutathione S-transferase π(GST-π) In Gastric Carcinoma And Their Correlations With Clinical Prognosis

Objective: To evaluate the expression of Epidermal growth factor receptor (EGFR) P-glycoprotein (P170), topoisomeraseⅡ(TopoⅡ) and glutathione S-transferaseπ(GST-π) in gastric carcinoma , their possible relationship with clinicopathologic parameters of tumors, and their prognostic significance.Methods: The expression of EGFR, P170, TopoⅡand GST-πin 160 patients with gastric carcinoma were detected by immunohistochemistry and the patients were followed up for more than three years. The relationship between the expression of EGFR, P170, TopoⅡ, GST-πand the clinicopathologic parameters were analysed. The relationship between these parameters and prognosis of patients were evaluated.Results: There was a positive correlation between the expression of P170 and GST-π, TopoⅡand EGFR. The expression of p170 were associated with tumor size, and the positive rates of p170 in patients with tumor size more than 5 cm in diameter were significantly higher than those less than 5 cm in diameter (P< 0.01). GST-πexpression were negatively related to the tumor differentiation, and the positive rate of ToPoII expression was markedly higher in lowly-differentiated carcinoma than that in highly- differentiated ones(P<0.05). In the tumor of deeper infiltrated, the expression of ToPoII was higher(P<0.05).The expression of P170 was positively related to the lymphatic lymphatic metastasis. EGFR, ToPoII, P170, GST-πwere significantly related to the distal metastasis(P<0.05). The expression of ToPoII,P170,GST-πtend to be higher in high TNM grade than that in lower ones(P<0.05). In the univariate analysis, age, tumor diameter, tumor site, histological type, depth infiltration, lymphatic metastasis, distance metastasis, TNM grade and the expression of EGFR,ToPoII,P170,GST-πwere related to the prognosis of gastric cancer(P<0.05). Multivariate Cox model analysis showed that the tumor site, histological type, depth infiltration, lymphatic metastasis, GST-πand EGFR were the independent prognostic factors for the patients (P<0.05). That means the tumor located in the proximal stomach, mucous adenocarcinoma, signet-ring cell carcinoma, undifferentiated carcinoma, lowly-differentiated carcinoma, and the tumor invade or penetrate the serous membrane, nodal status more than 7, over expression of GST-πor EGFR were the related the poor prognosis of the patients.Conclusion: the expression of EGFR,ToPoII,P170,GST-πwere related to some of the histopathological features, and some of these factors can be regarded as prognostic factors in gastric carcinoma. The further study of the prognostic factors will benefit to the diagnosis and treatment of the gastric carcinoma.
