Objective:To study the changes inβ—adrenoreceptors in heart and lymphocytes post—cardiopulmonary resuscitation in rats and the effect of Shen-fu onβ—adrenoreceptors and lymphocytes.And studyβAR density relativity between myocardial tissue and circulating lymphocytes and clinical significance.Methods:CA was induced by asphyxiation(succinylcholine) and ice-cold 0.5mol/L KCl in rats and resuscitation began five.minutes later.In this experiment,seventy-two male SD rats were randomizly divided into 9 groups:control group(sham group),Conventional recovery 0.5h,2h,4h,6h(common treatment groups)groups,and Shenfu intervention 0.5h, 2h,4h,6h groups(Shenfu treatment groups )(each group n=8).Except control group,rats of conventional recovery groups were allowed to reperfuse in NS in 20ml/h spontaneously while CPR,and rats of Shenfu treatment groups were allowed to reperfuse in high dose shenfu injection(20ml/kg) in 20ml/h spontaneously while CPR.Each rat was klled at the corresponding time and myocardial tissue and circulating lymphocytes were extracted respectively.3H—prazosln was used as a radioligand to determine changes inβ—adrenoreceptor in rat heart and circulating lymphocytes.The rat myocardial cellular apoptosis and injury were assessed by transmission electron microscope(TEM).Result:In control groups theβ-AR density in myocardial tissue and circulating lymphocytes was 194.42±8.43 fmol/mgpr and 201.43±15.24×10-6fmol/lym.In conventional recovery 0.5h groups theβ-AR density in myocardial tissue and circulating lymphocytes was 247.81±8.91fmol/mgpr and 312.34±21.31×10-6fmol/lym; theβ-AR density in myocardial tissue and circulating lymphocytes in 2h groups was 153.35±7.88fmol/mgpr and 163.74±18.58×10-6fmol/lym.And theβ-AR density in myocardial tissue and circulating lymphocytes in 4h groups was 104.17±13.87 fmol/mgpr and 113.2±14.8×10-6 fmol/lym.In 6h groups theβ-AR density in myocardial tissue and circulating lymphocytes was 178.53±10.42 fmol/mgpr and 188.25±13.46×10-6 fmol/lym.In Shenfu intervention 0.5h groups theβ-AR density in myocardial tissue and circulating lymphocytes was 259.91±18.09fmol/mgpr and 302.04±14.61×10-6fmol/lym; theβ-AR density in myocardial tissue and circulating lymphocytes in 2h groups was 173.04±18.77fmol/mgpr and 183.81±15.04×10-6fmol/lym.And theβ-AR density in myocardial tissue and circulating lymphocytes in 4h groups was 148.72±13.88 fmol/mgpr and 149.78±11.94×10-6fmol/lym.In 6h groups theβ-AR density in myocardial tissue and circulating lymphocytes was 191.32±20.54 fmol/mgpr and 198.54±17.28×10-6 fmol/lym. And no significant difference was found in the earlyβ-AR density between shen-fu treatment groups and conventional recovery groups(p>0.05),but shen-fu injection can significantly increase the latterβ-AR density after CPR in 2h,4h groups between shen-fu treatment groups and conventional recovery groups(p<0.05).Transmission electron microscopy showed the myocardial cell damage performance in rats after CPR:nuclear deformation,marginated chromatin,the nuclear membrane rupture, pyknosis,mitochondrial swelling,ridge fracture or even disappear,matrix smeared out, part of the vacuoles,crude surfaced endoplasmic reticulum expansion;There were visible morphological,accompanied by inflammation and necrosis on myocardial tissue slice.There was obvious correlation on theβ-AR density between myocardial tissue and circulating lymphocytes(r=0.892).Conclusion:Acute myocardial cells injury happened in rats after CPR because of hypoxia, ischemia-reperfusion injury et al.Because of the use of epinephrine and stress reaction,theβ-AR density in myocardial tissue and circulating lymphocytes after CPR was increasing in the early time after CPR,but out of continous hypoxia and ischemia-reperfusion injury, theβ-AR density began to decrease.Shenfu can improve theβ-AR density in myocardial tissue and circulating lymphocytes by anti-ischemia,anti-hypoxia,scavenging oxygen free radicals,anti-lipid peroxidation and reducing intracellular Ca2+ overloading,and hypertension,improve microcirculation et al,so it can improve success rate of CPR.The correlation on theβ-AR density between myocardial tissue and circulating lymphocytes can provide us theoretical basis in clinical by determinating theβ-AR density on people's circulating lymphocytes.
