Application Of The Combination Of Epinephrine And Vasopressin In Asphyxial Rabbit Models Of Cardiopulmonary Resuscitation

Objective: Timing is the crucial during CPR. It has been well identified that the earlier the spontaneous circulation restored, the better the outcome in heart arrested patients. In this study, we compared the diversity of resuscitation efficiency in asphyxial animals receiving different vasoactive agents. It is aim to investigate which would be the better choice of vasoactive agents for CPR.Methods Asphyxial rabbit model was used in this study. Forty animals were equally allocated in four groups. After basic life support, cardiopulmonary resuscitation , rabbits randomly received intravenous injection of normal saline , epinephrine (45ug/kg) , vasopressin (0.8U/kg) and epinephrine (45ug/kg) combined with vasopressin (0.8U/kg) . ROSC (Restoration of Spontaneous Circulation) , mortality within 2 hour, CPP and pHi were chosen as the endpoints in evaluating resuscitation efficiency.Results: ROSC appeared in 1,6,2, and 8 cases respectively in animals receiving normal saline , epinephrine, vasopressin and epinephrine combined with vasopressin. The mortality within 2 hours was the highest in normal saline group and the lowest in epinephrine combined with vasopressin (P<0.05) . The mean CPP in successful cases were significantly higher than that in the failed animals during CPR (P<0.01). Epinephrine combined with vasopressin showed the greatest effect on elevating CPP (P<0.01) . In surviving cases, the gastric intramucosal pH (pHi) in combination group were significantly higher than epinephrine group at 60 minutes and 120minutes after ROSC.Conclusion: There was close relationship between CPP and ROSC. During CPR in asphyxial rabbit models, the administration of epinephrine combined with vasopressin not only significantly increase CPP, ROSC and the rate of survive, but also improve splanchna perfusion.
