Objective: To evaluate therapeutic effect of Pancreatoduodenectomy with vascular resection for carcinoma of the head of pancreas and study the operative indication and model of carcinoma of the head of pancreas and to improve the resection rate and the survival.Methods: The clinical data of 223 patients with carcinoma of the head of pancreas were analyzed retrospectively from 1999 to 2005. 45 patients underwent PD, 14 patients underwent PD with SMV/PV resection, palliative hepaticojejunostomy was performed to the rest. In total 14 cases, 10 were performed lateral wall partial resection and reconstruction, 3 received segment resection and end to end anastomosis, 1 received segment resection of SMV /PV and artificial vascular graft.Results: All the patients who underwent palliative resection were died in 1 year after the operation, the 1 and 3-year survival rate of patients undergoing PD was 55.5% and 24.4% respectively, the 1 and 3-year survival rate of patients undergoing PD with vascular resection was 57.1% and 21.5% respectively. There are no difference in 1 and 3-year survival rate and the occurrence of postoperative complications(p>0.05).Conclusion: PD is the only way to cure carcinoma of the head of pancreas, PD with vascular resection can increase the resection rate and the survival on the condition of strict indication.
