Purpose:To approach the pathohistological and ultrastructural changes of lens epithelial cells of complicated cataract after silicone oil infusion,to analyze the difference of pathological changes about complicated cataract after silicone oil infusion and age-related cataract.Methods:1,Cataract extraction and silicone oil removal in 20 cases(20 eyes) were selected randomly as the experimental group,age-related cataract in 20 cases (20 eyes) were selected as the control group,which were choosen in our department in the period from March 2006 to October 2007.2,Both two group had undergone extracapsular cataract extraction or extracapsular cataract extraction,combined with intraocular lens ( IOL) implantation or secondary IOL implantation according to patient circumstance.As pathological specimen, anterior lens capsules were stained with 10% neutral formalin fixation and 3% glutaraldehyde fixation,which were observed under light microscope, scanning electron microscope(SEM) and transmission electron microscope(TEM).Result: 1,Observation under slit lamp: Subcapsular opacity and nuclear opacity were main in the lens in experimental group. There were three types cataracts in the control group:cortical cataract,subcapsular cataract and nuclear cataract.2,Observation under light microscope: In experimental group , lens epithelial cells(LECs)were irregularly shaped, with flat or hypo-collumn. Extracellular spaces were of cystic dilatation, vacuolization,parts of epithelial cells exfoliating.Compared with the experimental group, similar changes were also observed in the control group, but not obvious.3,Observation under electron microscope:(1)Observation under SEM: Anterior lens capsule crispatured in both of groups.In experimental group,parts of endothelial cells and ECM deprivated anterior lens capsule ,and the lids side of LECs covered the top of flanking cell, endothelial cells were flat and cubic,and cell nucleuses were round or oval.there were broadly similar appearance both the experimental group and control group , but the LECs and ECM ,missing or deprivated anterior lens capsule ,were not serious in control group.(2)Observation under TEM:In experiment group, anterior lens capsules were homogeneous basement membrane,the shape of LECs were extremely irregular shape, spindle or flat,and cell conjugation were raritas,and there were multiplicity of inequality of size vacuolus. And among LECs were tight or gap junctions,with chondrosome swelling and cristae disappearing. Cellular nucleus were ellipse,deep density,and chromoplasm condensation, margination, shrinkage,which were typical of apoptosis performance. vacuolus of LECs among the control group had small changes compared which the experiment group, apoptosis was main to the early performance.Conclusion:Compared with age-related cataracts,it was more evident in pathology changes of complicated cataract after silicone oil infusion.In the ultrastructure of the latter's LECs, there was the capsule-like expansion of large extent,the more obvious and typical of apoptosis.
