Clinical Trial On Treatment Of Acute Paraquat Oral Poisoning

Objective Paraquat is widely used in the world,and all treatments for paraquat poisoning have been unsuccessful.Over the last decades,there have been numerous fatalities mainly caused by accidental or voluntary ingestion.To investigate the clinic therapeutic effect of methylpred-nisolone combined with cyclophosphamide and Etanercept method on acute paraquat poioning.Methods All 136 cases acute paraquat oral poisoning patients were divided in normal therapy group and intensive therapy group randomly.Methylprednisolone, cyclophosphamide and Etanercept was used in intensive therapy group. Methylprednisolone 500mg was given intravenous per day for continuous three days followed 200mg intravenous per day.Then decrease methylprednisolone gradually 14d or 21d later according to the patient's condition.Cyclophosphamide 600mg was given intravenous twice weekly for 3 weeks and Etanercept 25mg was given hypodermic injection twice weekly for 3 weeks.Curative effect evaluation was done at 7d,14d,21d and 12week after therapy.Results The survival rate of intensive therapy group was obviously higher than that of normal therapy group(P＜0.01)on 7d,4d,21d and 12 weeks.The cure rate of intensive group were 94.6%(intake dose＜50ml 20%paraquat solution),75.0%(intake dose 50ml～100ml 20%paraquat solution),12.5%(intake dose＞100ml 20%paraquat solution),while cure rate of the normal group were 16.7%(intake dose＜50ml 20% paraquat solution),8.3%(intake dose 50ml～100ml 20%paraquat solution),0%(intake dose＞100ml 20%paraquat solution).The total cure rate of intensive therapy group (78.3%)12 weeks later was higher than that of normal group(11.9%). Conclusions Methylprednisolone combined with cyclophosphamide and Etanercept intensive therapy has curative effect on acute paraquat poisoning.
