Objective To evaluate the catheter-directed thrombolysis combined with surgery and intraluminal treatment in treatment of deep venous thrombosis (central type).Method A total of 111 lower extremity DVT patients (115 limbs)that the course of disease is between 7h-2m .111 lower extremity patients are divided into 4 groups. Group A: 10 patients, catheter-directed thrombolysis combined with surgery and IVCF implanting, group B: 5 patients catheter-directed thrombolysis combined with surgery and IVCF implanting and intraluminal dilating and stenting in the iliofemeral vein. Group C: 19 patients catheter-directed thrombolysis combined with IVCF implanting and intraluminal ballon dilating and stenting in the iliofemeral vein, group D: 76 patients catheter-directed thrombolysis alone. The clinical therapeutic effect and short follow-up outcome were analyzed.Result Group A 4 is typeâ…¢(thrombosis recanal type), 2 patients is typeâ…¡b, 4 patients are typeâ…¡a, Group B: 4 patients is typeâ…¢, 1 patient is rethrombosis in the iliofemeral stent. 1 patient is thrombosis on the IVCF. Group C: 17 patients is typeâ…¢, 1 patient is rethrombosis in the iliofemeral stent. 1 patient is restenosis after ballon angioplasty.(stenosis >80%). Group D: 18 patients is typeâ…¢, 40 patients isâ…¡b type, 13 patients isâ…¡a type, 5 patients is type I .3 patients is complication when thrombolysis , and come to normal after energetic treatment.Conclusion catheter-directed thrombolysis with transpopliteal vein access for the treantment of lower extremity DVT is safe and effective , combined with surgery and intralunminal treatment can rapidly recover the blood stream of deep vein.
