Clinical Study On Deng's Guanxin Capsule For Treatment Of Contrast Medium-induced Nephrotoxicity After Percutaneous Coronary Intervention Of Diagnosis And Treatment Of CAD Patients

ObjectiveTo study curative effect of the Deng' s Guanxin Capsule for Treatment of Contrast medium-induced nephrotoxicity(CMIN)after percutaneous corona ry intervention of diagnosis and treatment of CAD patients,and to provide a practical and valuable Chinese medicine approach for clinical practice.MethodsBase on the Blind Method randomization principle,40 patients were divided into two groups and underwent two weeks treatment,the treatment group was prescribed Deng's Guanxin Capsule and mainstream therapy,while the controlled group was only prescribed mainstream therapy.Adopting serum Cys C,urineβ2-MG as laboratory targets,two groups were observed the improvement of study before and after the treatment,and contrast the appraisal of the clinical curative effect improvement situation between two groups.Results1.The increased rate of Scr level is(4.31±9.22)%,there was 21 cases which the rate more than 5%,and CMIN in the rate of 52.5%;The most case of CMIN was happened between the rate of 5%to 8%.2.Two groups before treatment in general,the angina points,coronary artery disease,nephrotic TCM Syndrome points classification and the renal function targets,there were no significant difference(P＞0.05).3.After two weeks treatment,two groups had no distinct differences, referring to angina Points,the Improve rate of angina Points and coronary artery disease TCM Syndrome points(P＞0.05).4.The Scr,Cys C and urineβ2-MG level of treatment group had signifi cant differences before and after treatment(P＜0.05),and the treatment group did has improved on renal targets.However The control group was not obvious difference(P＞0.05).5.Two days after intervention,there was no significant difference in the Contrast effection for nephrotic TCM syndrome classification(P＞0.05);D14 days after treatment,the nephrotic TCM syndrome classification of treatment group had significant differences(P=0.001),but the controlled group not. The result shows that Deng' S Guanxin Capsule had obviously effected to nephrotic TCM syndrome.6.The related analysis of renal targets showed:Scr and Cys C were negatively correlated with Ccr,Correlation coefficient r cys＞rscr;Urineβ2-MG level was positively correlated with Ccr,r to 0.177.It means serum Cys C and urineβ2-MG has good associated with Ccr.7.Around the treatment,all Renal targets in different levels of nephrotic syndrome classification had no significant difference between two groups(P＞0.05),and also in different levels of heart function classification.8.Both Two groups had no significant side effects,and the safety and tolerability are good.Conclusions1.The Sensitivity and Specificity of CysC andβ2-MG overmatch BUN and Scr. They are good target of the renal function degree.2.For the CAD patients of Qi deficiency with phlegm and blood stasis, Deng's Guanxin Capsule has a significant curative effect on treatment of CMIN after percutaneous coronary intervention of diagnosis and treatment,could be protect the kidney function by improving blood Cys C and urineβ2-MG.3.Deng's Guanxin Capsule has a good safety and tolerability.It is worthy of further research and development.
