Objective:To study the relationship between the quantity of hepatocytes HBV cccDNA and tDNA,HBsAg,HBcAg in the hepatocytes,and HBV DNA,HBsAg,HBcAg in the serum in chronic hepatitis B patients and the clinical application。Methods:The research included the four groups:CHB group(28 chronic hepatitis B patients),PLC group(30 HBV liver cancer patients),antiviral group (4 patients with chronic hepatitis B after nucleosides therapy over 2 years reaching the China criteria of discontinuation)and control group(7 non-hepatitis patients with negative HBV marks and HBV DNA in serum).The quantity of HBVcccDNA and tDNA in the hepatocytes and HBV DNA m the serum of all four groups were assayed by FQ-PCR,and the level of sera HBsAg,HBeAg were detected by ELISA,the express of HBsAg,HBcAg in the hepatocytes identified by immunohistochemistry.The correlation between quantity of hepatocytes HBV cccDNA and tDNA,HBsAg,HBcAg in the hepatocytes,and the sera HBV DNA,HBsAg,HBcAg in chronic hepatitis B patients were analysesed with spss 13.0.Results:(1)There are no significant differences between the quantity of HBV cccDNA,tDNA in the hepatocytes with different expressive mode of HBsAg,HBcAg.(P=0.128,P=0.111).(2)There was low positive correlation between the the quantity of HBV cccDNA in the hepatocytes and sera HBV DNA(r=0.387,p=0.003).(3)There were a low positive correlation between the quantity of HBV cccDNA in the hepatocytes and sera HBsAg(r=0.295, p=0.025),but no correlation between the quantity of HBV cccDNA in the hepatocytes and sera HBeAg(r=0.125,p=0.254).(4)There is a high positive correlation between the quantity of HBV cccDNA and HBV tDNA in the hepatocytes(r=0.881,p=0.00).(5)No significant differences among the quantity of hepatocytes HBV cccDNA in the CHB group and the liver cancer cells and the cells by the liver cancer in PLC group,p=0.459.(6)The quantity of HBVcccDNA and tDNA in the hepatocytes,the quantity of sera HBV DNA and HBsAg,HBeAg in antiviral group decreased than that of the 12 patients without antiviral therapy,but the HBVcccDNA and tDNA in the hepatocytes in antiviral group could be detected yet.Conclusion:(1)There is no regularity of the correlation between the quantity ofHBV cccDNA,tDNA in the hepatocytes and the expressive mode of HBsAg,HBcAg。(2)When HBV cccDNA transcription was active the quantity of HBV cccDNA,tDNA in the hepatocytes and sera HBV DNA increase correspondly,when HBV cccDNA transcription was inactive,even though HBV DNA<10~3 copies/ml,HBV cccDNA in the hepatocytes might still be detected at low load;(3)The clinical significance and academic value of the quantity assay of HBV cccDNA in the hepatocytes could not be substituted by the detction of hepatocytes HBV tDNA and sera HBV DNA,HBsAg and HBeAg;(5)Even though patients accepted nucleoside analogue therapy over 2 years and satisfied the criteria of therapy endpoint in China guideline for hepatitis B patients,their cccDNA could be found at low load in the hepatocyteso(6)The HBV cccDNA in the hepatocytes could be exhausted after Long-term(>2years)antiviral therapy with nucleoside analogue.
