| Part one Experimental research of CT-guided thermochemotherapy for pig's healthy pancreasObjective: To investigate the safety and efficacy of CT-guided pancreatic carcinoma by CT-guided thermochemotherapy for pig's healthy pancreas.Methods: 12 healthy pigs were treated by CT-guided 5-FU thermochemotherapy, The treated pigs were scanned by CT and MRI and phlebotomized 10ml for testing AMY at 12h,3 day,7 day and 14 day after treated, and 12 pigs were sacrificed at 3 day,7 day and 14 day respectively to get the pathology sample. Then compare the result of AMY value, the lesion size and HE condition.Results: 1h after 5-FU was injected, the volume of pancreas tail was tumefacient, panrenchyma density was decline and unequal, peripancreatic fluid shadow was seen. According the CT scanning results, the biggest local lesion size were founded at 3-day after treated, 1h after treated were the bigger, then were the 7-day and 14day respectively, which was confirmed by the pathology samples. The change of AMY value and the necrosis size was accordant. At 14 day after treated, the lesion size was reduced obviously, Only one pig was diagnosed by AP. No serious complications were found in other 8 pigs. The lesion size of 1h after being treated was the biggest, then with time passing by , the size of the lesion become smaller and smaller.Conclusion CT-guided thermochemotherapy can lead to obvious necrosis with minimal damage, For advanced pancreatic carcinoma, CT-guided thermochemotherapy should be considered because its safety and efficacy. Part two Experimental research of CT-guided raidofrequency ablation for pig's healthy pancreasObjective: To investigate the safety and efficacy of CT-guided pancreatic carcinoma by CT-guided radiofrequency ablation for pig's healthy pancreas.Methods: 12 healthy pigs were treated by CT-guided radiofrequency ablation,The treated pigs were scanned by CT and MRI and phlebotomized 10ml for testing AMY at 1h,3 day,7 day and 14 day after treated, and 4 pigs were sacrificed at 3 day,7 day and 14 day respectively to get the pathology sample.Results: 1h after 5-FU was injected, the volume of pancreas tail was tumefacient, panrenchyma density was decline and unequal, peripancreatic fluid shadow was seen. Part three The comparision between Experimental research of CT-guided thermochemotherapy and radiofrequency for normalpig's pancreasObjective: By CT-guided thermochemotherapy and raidofrequency for pig's normal pancreas, discussing and evaluating two ways' safty, feaisbility, efficacy and damage mechanism.Method: 24 healthy pigs were randomly divided into two groups, each group has 12 pigs, one group were treated by CT-guided 5-FU thermochemotherapy, the other group were treated by CT-guided radiofrequency ablation on the pancreas tail. The treated pigs were scanned by CT and MRI at 1h,3 day,7 day and 14 day after treated in order to get the condition of necrosis size, they wer also being phlebotomized 10ml for testing AMY at 12h,3 day,7 day and 14 day after treated. and 12 pigs were sacrificed at 3 day,7 day and 14 day respectively to get the pathology sample and be HE died to see the necrosis under microtelescope. Each group has one pig which was diagnosed pancreatitis because of their high AMY level. Then do statistical analysis among the result of AMY value, the necrosis size and HE condition under microtelescope.Result:â‘ Statistical amalysis of AMY level between corresponding period: using SAS statistical analysis software, there were no significance statistical difference between baseline, 12h after being treated and 7d after being treated of each group, P>0.05; But there was significance statistical difference between 3d after being treated of each group, P <0.05.â‘¡Statistical amalysis of necrosis size between corresponding period: there were significance statistical difference between 1h , 3d, 7d and 2w after being treated of each group, P<0.05.â‘¢Statistical amalysisof complication rate: there was no significance statistical difference between two groups.â‘£Comparision among necrosis under microtelescope: Clear necrosis were found in all HE died pathology specimen under microtelescope. Hemorrhage and necrosis was found in pancreas tissue and fat tissue 3d after being treated by two ways, plenty of WBC were found, too. with the time passing by, the number of WBC was decline, and fibrous tissue hyperplasia.Conclusion:â‘ The AMY level of 3d after being treated in RFA group was higher than that in thermochemotherapy group, there was statistical difference between the two group(P < 0.05), so the pancreatitis was more possible in RFA than that in thermo chemotherapy;â‘¡compare with CT-guided thermo chemotherapy, one-time CT-guided RFA can make larger necrosis size, so, CT-guided RFA should be more fitting for larger tumor treating. But the relationship among the tumor size, treating time and treating area, and how to controll the treat temperature should be done more research.â‘¢the complication rate of each group was the same and was low, one pig of each group was proved be high AMY level, pancreatitis was considered priority. So, these two ways should be do more research and worthy of being applied in clinical. |
