ObjectiveTo analyze the important factors affecting the prognosis of patients with gastric carcinoma and study the expression of p53 protein in human gastric carcinoma and the relation to clinicopathology and prognosis.Methods1. Data collection Collected 132 cases of patients with gastric cancer resected in our hospital from June 2004 to October 2004.The expression of p53 protein in all of the gastric tissues were detected by immuno- histochemistry technique after operation. 104 cases of patients were followed-up by telephones, correspondence and outpatient following-up visits. The result of the 104 tissues of gastric carcinoma were studied.2. Statistics methods All data were analyzed by SPSS13.0.The stati- stics methods used include Chi-square test,Kaplan-Meier,log一rank test and Cox Regression.A P-value less than 0.05 was considered to be statistically significant.Results1. For all cases,the 1-,2- and 3-year survival rates were 67.3%,61.5%,50%.2. The patients with the positive expression of P53 protein in tissues of gastric carcinoma were 48 cases,accounting for 46.2%.While the patients with the negative expression of P53 protein were 56 cases,accounting for 53.8%.3. There were significant differences between different infiltration depth and regional lymph node metastasis in the expression levels of P53 protein,while there were not significant differences between patient's sex,age, tumor size, location, histological type,distant metastasis,nor pTNM stage.4. The independent risk factors influencing prognosis of these patients with gastric carcinoma were TNM staging,curative resection,chemotherapy after operation and the expression levels of P53 protein through uni- variate and multivariate analyses.5. The equation of risk factors for prognosis of patients with gastric carcinoma was founded,h(t,x) =h0(t) exp (0.406x1+0.796x2- 0.603x3+ 0.391x4).Prognostic index is equal to 0.406x1+0.796x2- 0.603x3+ 0.391x4。In this equation, x1 stands for TNM staging,x2 stands for the type of resection,x3 stands for chemotherapy after operation,x4 stands for the expression levels of P53 protein in tissues of gastric carcinoma.Conclusions1 The expression levels of P53 protein in tissues of gastric carcinoma both closely correlated with infiltration depth and regional lymph node metastasis.2 The factors influencing prognosis of these patients with gastric carcinoma were TNM staging,curative resection,chemotherapy after operation and the expression levels of P53 protein.
