Objective:To explore the role of apoptosis correlated molecules Fas, Cytc, Caspase-8, Caspase-9, Caspase-3 in CA1 field of the ipsilateral hippocampus to the seizure of MTLE.Methods:The experimental were 30 patients with typical mesial temporal lobe epilepsy (MTLE) , while control group were six patients with vascular malformation in temporal lobe , which were operated and all from FuZhou General Hospital in 2007.3 to 2008.3. Neuron apoptosis imformations in CA1 field of the ipsilateral hippocampus to the seizure of MTLE was investigated by hematoxylin and eosin stain and Terminal deoxynucleotidyl Transferase-mediated dUTP Nick End Labeling (TUNEL) method and overviewed under light microscopy; The expressing levels of the molecules Fas, Cytc, Caspase-8, Caspase-9, Caspase-3 in CA1 field neurons of the ipsilateral hippocampus to the seizure of MTLE were compared to those of through Immunohistochemistry, relationship and difference were analysed among these molecules; used Magnetic Resonance Spectrum techniques to record the ratio of NAA/(Cho+Cr) in the hibateral head of hippocampuses in MTLE patients and accout decreased extent of the ratio of NAA/(Cho+Cr) in the ipsilateral hippocampus to the seizure of TLE patients compared with the contrallateral subgroup, analysed relationship between them and the expressing level of Caspase-3; analysed relationship between the expressing level of Caspase-3 and course of disease, frequency of epileptic seizure.Results:The phenomenance of neuron cataplasia and phagocytizing was observed in the smears with hematoxylin and eosin stain and 18 smears with Terminal deoxynucleotidyl Transferase-mediated dUTP Nick End Labeling (TUNEL) method were positive in MTLE patients. Compared with control group, The six proteins'immunoreactivities was found obeviously but few neuron in control group, the differences were statistically significant (P<0.01). Protein Fas,Caspase-8,Caspase-3 immunoreactivities were increased statistically relatively and so were Cytc protein,Caspase-9 protein,Caspase-3 protein,The level of fas protein expressed was higher than it of Cytc, and the level of Caspase-8 protein was also higher than of Caspase-9(P<0.01).And the level of caspase-3 protein was stastically relative with decreased extent of the ratio of NAA/(Cho+Cr) in the ipsilateral hippocampus to the seizure of TLE patients compared with the contrallateral subgroup. The expressing level of Caspase-3 was stastically relative with frequency of epileptic seizure not course of disease.Conclusions:Neuron apoptosis is involved in mesial temporal lobe epilepsy. Neuronal cell death may occurs via not only intrinsic (death receptor) but also extrinsic (mitochondria) pathways, the intrinsic (death receptor) pathway may be a main one.Apoptosis may lead to the neuron loss which is the character in pathology of MTLE patients in some extent, while intrinsic apoptosis may be basic reason for epilepsy and extrinsic apoptosis may responsible for epilepsy development. There may be a iterative cycle of apoptosis, pathological change, epilepsy seizure, apoptosis, which was the reason than MTLE is difficult to controlled.
