Object i ve To analyze the influencing factors of abnormal liver function and DNA damage in vinyl chloride workers.Methods The excessive activity of serum alanine aminotransferase (ALT) and abnormal manifestation of liver B-ultrasound were used as the parameters of liver damage. According to the health examination data over the years, we studied the influential factors from the following aspects: hereditary factors(gene polymorphism of vinyl chloride metabolic enzyme), living habit (smoking, drinking) and vinyl chloride performance. Comet assay was employed to detect peripheral blood lymphocyte DNA damage. The genotypes of GSTT1, GSTM1 and CYP2E1 were determined by polymerase chain reaction (PCR) and/or restriction fragment length polymorphism (RFLP) on lymphocyte DNA. With workers' health examination, the experimental results were analyzed.Results Multiple Logistic regression analysis showed CYP2E1 clc2/c2c2(OR=10.955, 95%CI: 3.144-38.169) and drink habit (χ~2 =10.323, P=0.006) were associated with the abnormal liver function. Spearman rank correlation analysis showed the average VC exposure dose(r_s=0.243, P=0.014) correlated with level of plasma enzymes (ALT) . Multiple logistic regression analysis showed that CYP2E1 c1c2/c2c2(OR=4.253, 95%CI: 1.247-14.500), age(OR=1.192,95%CI: 1.007-1.411), and spearman rank correlation analysis showed that age (r_s=0.276, P=0.015) , the average VC exposure dose(r_s=0.248, P=0.030) ,and rank sum test drinkhabit(χ~2 =7.580, P=0.023), could be associated with the DNA injury.Conclusion The abnormal liver function and DNA damage are influenced bygenotypes of CYP2E1, drink and the average VC exposure dose. We should pay attention to the VC workers' liver damage from the aspects such as hereditary factors, living habit and vinyl chloride performance. The health care of VC workers should be given full consideration.
