Objective:Otitis media with effusion is the result of varied factors interactived. In the initial stage,the studies indicated that adenoid located in median line of roof of the nasopharynx's posterior wall,the hypertrophic adenoid closed together torus tubalis and pharyngeal opening of auditory tube.When swallowing or yawning,the hypertrophic adenoid inter-crushed with palatum mobile,the levator veli palatini abnormally contracted.As a result,the pharyngeal opening of auditory tube didn't open normally.[1]The child patients whose adenoid are hypertrophy often accompany with nasal obstruction,they often emphaticly blow nose.So eustachian tube often in the condition of negative pressure.This condition will cause the middle ear effusion,otopiesis and so on.But Gates[2]and others considered that the size of adenoid itself didn't have obviously associativity with the pathogenesy of OME(otitis media with effusion).The pathogenesy was the active function of adenoid,and then it induced the heavy releasing of beta-imidazolyle thylamine,peripheral vasodilation,permeability raising up and eustachian tube mucosa oedema;the result was that eustachian tube function became disturbance and then induced OME.For the past few years,along with the rapid development of molecular biology and detection technology,a platoon body fluid transmitter-cytokine was just beening reconstructed increasingly by people in the pathogenesy of otitis media with effusion. Cytokine could be a very important factor in the pathogenesy of otitis media with effusion.Wang Zhi nan[3]had reported that the cytokine of IL-6,IL-8 was significant high in middle ear effusion than that in the blood plasm in child patients with OME compared with healthy children,he found that the content of IL-6,IL-8 in the blood plasm with OME was significant high than that in heathy group[4,5].But the past clinical observations told us that the adenoidal hypertrophy was not often accompanied with otitis media with effusion in child patients.With theose we conclude that some cytokine may be a very important factor in the pathogenesis of otitis media with effusion.Yellon(1991)had certified that in the early days of otitis media with effusion the IL-6 and IL-8 participated in the defence reaction of organism,and enhancemented the creation of serosity middle ear effusion.Through detecting the content of IL-6,IL-8 in the adenoidal organism and blood plasm in the group of otitis media with effusion,and compaired with the child patients of simple adenoid hypertrophy,we could find the relationship among the changes of adenoid immunity or humoral immunity and the pathogenesy of OME.We expected to find a new way to aim at the pathogenesy of OME and the healing of child patients. Method:The content of IL-6,IL-8 were detected in the adenoid and blood in the child patients of simple adenoid hypertrophy and accompany with otitis media with effusion by ELISA.The specific steps was as follow:1The election and treatment of experiment sampleThe child patients who suffered from adenoid hypertrophy and accompanied with otitis media with effusion were selected.The questionnaire,examination of snuff lateral projection,Nasal endoscopy,examination of cavum tympani fluidify,examination of acoustic immitance and pure tone audibility threshold had been done in those child patients. According to the examination consequences the child patients were separated to two groups:the group of the adenoid hypertrophy accompany with otitis media with effusion and the group of simple adenoid hypertrophy.Ten patients had been selected from each group.The adenoid of child patients were ectomized in general anaesthesia,and their blood were hemospasiaed.Then their blood were centrifugated,and we obtained serum.At last the adenoid organism of child patients and serum were seted in the freezer of -70℃.2The preparation and detection of the sampleThe sample were defrosted in common temperature and attenuated by destillata in 1:20,and then were homogenated,centrifugated,at last obtained the clear supernatant liquid.The content of IL-6,IL-8 of the samples which in two groups were detected by ELISA. 3Statiscs managementCompletely randomized one-way analysis of variance and Student-Newman-Keuls test were used to analyze the data.α=0.05 was considered as significant difference.Result:The expression of IL-6 and IL-8 in the adenoid organism and blood serum of the group of simple adenoid hypertrophy and OME.1The expression of IL-6The content of IL-6 in the adenoid organism of the group of simple adenoid hypertrophy was(x_±s):13.3347±0.862123; The content of IL-6 in the adenoid of the group of otitis media with effusion was(x_±s):16.3212±0.983563;The content of IL-6 in the blood serum of the group of adenoid hypertrophy was(x_±s):3.3533±0.683575;The content of IL-6 in the blood serum of the group of otitis media with effusion was(x_±s): 3.9325±1.001492.The each two groups were compared:1.1The expression of IL-6 was significantly different in adenoid organism between the group of simple adenoid hypertrophy and the group of accompany with otitis media with effusion(P<0.05).1.2There was a significant difference between adenoid organism and blood serum in the group of simple adenoid hypertrophy (p<0.05).1.3There was a significant difference between adenoid organism and blood serum in the group of OME(p<0.05).1.4There wasn't a significant difference in the blood serum between the group of simple adenoid hypertrophy and the group of OME(p>0.05).2The expression of IL-8The content of IL-8 in adenoidal organism of the group of simple adenoid hypertrophy wa(sx_±s):101.986±5.5609399;The content of IL-8 in the adenoid of the group of otitis media with effusion was(x_±s):99.8285±5.4376508;The content of IL-8 in the blood serum of the group of adenoid hypertrophy wa(sx_±s): 99.9673±7.41553753;The content of IL-8 in the blood serum of the group of otitis media with effusion was ( x_±s ) : 100.2190±4.02589289.The each two groups were compared:2.1There wasn't a significant difference of the expression of IL-8 in adenoid organism between the group of simple adenoid hypertrophy and the group of accompany with otitis media with effusion(P>0.05).2.2There wasn't a significant difference between adenoid organism and blood serum in the group of simple adenoid hypertrophy(p>0.05).2.3There wasn't a significant difference between adenoid organism and blood serum in the group of OME(p>0.05).2.4There wasn't a significant difference in the blood serum between the group of simple adenoid hypertrophy and the group of OME(p>0.05).Conclusion:Through the experiment we discovered that cytokine was a very important factor in the pathogenesy of otitis media with effusion.IL-6 was very correlatively with the pathogenesy of otitis media with effusion and the partial immunologic abnormality of the adenoid which IL-6 caused would be a very important factor in the pathogenesy of otitis media with effusion.
