A Comparative Study Between The Improved PRK And The Traditional PRK By The Alcohol Removing The Cornea Epithelial Flap In The Rectification Of Myopia

Objective: compare the effect between the improved PRK and the traditional PRK by the alcohol removing the cornea flap in the rectification of myopia .Methods: Choose 136 examples of myopia patients (272 eyes), 64 examples men (128 eyes), 72 examples women (144 eyes), 19-40 years old , average age 27.2 years old, the isoeffect sph,:right eyes -7.53±1.23D, left eyes -7.67±1.43D, the isoeffect cyl: right eyes -1.13±0.47D, left eyes -1.45±0.37D, central corneal thickness of right eyes 491.23±15.37μm, and left eyes 4789.31±14.21μm; perform the traditional PRK on the right eyes (control group), the traditional PRK on the left eyes (experimental group); recipe the free corneal flap in operation to do HE staining, observe the succession of corneal basement membrane of epithelium and assessment the integrity of removed corneal epithelium;after one day, two days, three days, four days, five days, 2 weeks, 1 month, 3 months, 6 months , observe the postoperative pain,the time of corneal epithelial healing,vision,refractive status,the degree of corneal haze,corneal topography,wavefront aberrations and the succession of corneal basement membrane of epithelium.result: On the 1th day postoperatively ,there are 91 eyes(66.9%) pain which is the first grade in the improved PRK, while 74 eyes(54.4%) pain which is the second grade in the traditional PRK, the difference is significant (P <0.05).;the condition of corneal epithelial healing: corneal epitheliums of two group patients have been healed after 2-4 days , the time is 2.66 da ys averagely in the group of the improved PRK while 3.21 days in the group of the traditional PRK, the difference was significant (P <0.05). Visual acuity after the third day≥1.0: PRK improved 19.8 percent of the 27 cases, 7 cases of traditional PRK 5.1% (P <0.05> statistically significant difference; 3,6 months after the first PRK Group haze improved significantly lighter than traditional PRK (P <0.05); After two weeks, one month, three months, six months traditional PRK and improved PRK visual acuity and best corrected visual acuity has no significant difference (P> 0.05); Improvement after the first January-PRK smooth topography 72.1% of 98 cases, the traditional smooth-PRK 43.4% of the 59 cases, there were significant differences. PRK improve the higher-order aberrations than conventional PRK group (P <0.05); PRK improved in the group of corneal epithelial integrity of the production better than traditional PRK group (P <0.05).Conclusion: The improved PRK by the alcohol removing the cornea flap has a satisfactory therapeutic efficacy . It is superior to the traditional PRK such as the postoperative pain,corneal epithelial healing,the occurrence of haze,the morphous of corneal topography and higher-order aberrations.
