Tuberculosis ( TB) ,known as "white plague, "is a chronic infectious diseases which can make serious hazard to human health. China is one of the 22 TB high-burden countries in the world and the number of tuberculosis patients ranks 2nd.The core of TB control work is to detect in time and cure TB patients completely. National tuberculosis epidemiological random sampling survey in 2000 showed that there were nearly 2/3 of active tuberculosis cases and nearly 3/5 of the smear-positive TB cases undetected, only 29.6 percent tuberculosis patients were diagnosed before the survey, more than 79% patients got irregular treatment in the country. TB found delay and irregular treatment has become an important influence factors which increase the spread of the disease and exacerbate the suffering of patients, and further, the latter could lead to Multi-drug resistance of tuberculosis. TB found delay and cured delay were reported more at home and abroad, but none was reported on large-scale industrial and mining enterprises till now. Shengli Petroleum Administration Bureau is a large oil company in our country,whose TB incidence rate is in a higher level now despite a sharp drop by 30 years TB control. The TB control of Shengli group has its own particularity: the workers and their families live scattered because of Oilfield's wide geographical distribution and the construction site at the whole country, people transfer from one place to another frequently. From the nature of work and lifestyle, some similar to urban residents,some similar to the remote rural workers,some has no fixed abode in the drilling,and others work on shallow water and sea operations. The group is so complex which make the TB detection and enforcement of DOTS become more arduous and complicated. For this reason,this study use tuberculosis patients who take cure in the Shengli Petroleum Administration as the object of study,through collecting the demographic characteristics, prevalence of TB,medical seeking,medical treatment and other influencial factors,interview the administrative staff and health professionals who work in health administration departments and Medical and health institutions to acknowledge the situation.Patients delays, delays in diagnosis and treatment were analyzed, The relationship between relevant factors and delays in diagnosis and treatment was inferred with chi-square testing and non-conditional logistic regression model.The study focused on the influential factors on delay in diagnosis and treatment of tuberculosis patients in the Shengli Oil field,and lay a basis for making reasonable suggestions and measures for TB control in Shengli Oil field and palces of similarities.Results:1. The new smear positive tuberculosis patients' registration rate was 25.4/100000 and cure rate was 94%,while registered tuberculosis patients mortality rate was 0.6/100000 in Shengli Oil field in 2006. The new smear positive tuberculosis patients'registration rate was 33.8/100 000 and cure rate was 95.6 %,while tuberculosis mortality rate was 1.9/100000 in Dongying City at the same time.2. 214 cases of tuberculosis treatment were delayed on an average of 28.37 days, 72 patients(33.6%) delay in treatment more than 14 days; the average delayed diagnosis days were 39.69, 72 cases (33.6%) assumed delay;average delay in treatment were 30.92 days, 30 cases (14.0%) confirmed delay in treatment.3. 37.9% of patients received knowledge of tuberculosis disease before detected. all levels of medical institutions check on sputum smear of tuberculosis patients was not enough.4. Influence factors of patients delay:The patients who belong to oil field, get more income, never got health education on TB , who are absence of haemoptysis and extra-pulmonary TB seems to have a higher chance to patient delay.5. Influence factors of health system delay:The patients who belong to oil field, older,married,lower level of education,lower first health facility seems to be significantly associated with a higher risk of health system delay.6. Influence factors of treatment delay:People who are female,neither dwell in nor work in Dongying,need more than one hour to reach in the center of TB control,got final diagnosis in health facility of oil field and extra-pulmonary TB appear to have a lower chance to delay for treatment.Conclusions:1. Medical visit of all tuberculosis were delayed an average of 28.37 days, 72 cases (33.6%) delayed;the new tuberculosis patients in an average of 27.70 days delay, 46 cases (28.9%) delayed.2. More patient delay happen in oilfield patients and extrapulmonary tuberculosis patients, on higher-income individuals, who didn't get the publicity of knowledge of tuberculosis before detected, with no symptoms of hemoptysis.3. TB patients of Shengli oil field are easier to access to the health system because of convenient transportation.4. Tuberculosis control efforts are not enough at publicity of knowledge;such as on television, newspapers, networks,etc.the use of mass media and propaganda field.5. Clinician awareness of tuberculosis needs improving,,it is suggested to do microscopy inspections in suspicious TB cases seeking treatment.
