Objuctive:The anterior mediastinum is a unique anatomic area containing several structures and pluripotent cells that allow for the development of a range of malignant tumours. They are similar in radiological manifestation and genesis location but different in treatment. Methods: 12 patients of primary anterior mediastinal malignant tumours, surgically intervened and pathologically confirmed in Shandong Provincial Hospital through November 2006 to February 2008, were retrospectively analysed. Among the total 12 patient, thymic tumours 8 cases, Germ-cell Tumours 2 cases, lymphoma 2 cases. 6 patients underwent gross total resection, 5 had subtotal resection, and 1 exploratory thoracotomy and biopsy alone. Results: In this series there was no perioperative death. The pathological findings were thymic mucoepidermoid carcinoma in 1, thymic squamous cell carcinoma in 4, thymic neuroendocrine carcinoma in 1, thymic unrepresentative carcinoid in 1, invasive thymoma in 1, yolk sac tumor in 1, immature teratoma in 1, Hodgkin lymphoma in 1, large-cell lymphoma in 1. Through 3 to 17 months follow-up, there was no postoperative recurrence, metastasis and death. Conclusions: Many kinds of malignant tumours can occur in the anterior mediastinum, the first choice of treatment is total resection, and operation will be only for biopsy or the extenuation of symptom when it is impossible. But it is necessary to excis the tumour as much as we can. Combined therapy include surgery can bring us a better prognosis. |
