Part One:Background On Rheumatoid Arthritis(RA)Rheumatoid arthritis(RA) is a chronic autoimmune disease,which is characterized by symmetry and destructive joint impairment.RA has many inducing factors,of which pathogenesis is complex.It is commonly considered that it is caused by genetic and environmental factors.According to statistics,about 90%of RA patients were sensitive to climate change;the recent study also showed that RA were closely related to the change of weather factors.RA is not only a seasonal disease(onset of a season),but also a weather disease(meteorological factors dramatically change in the short time).This study,from seasonal disease and weather disease two respects,deeply investigated the connection between RA and environment.Part Two:Influence of Meteorological Factors on Arthralgia in Rheumatoid Arthritis PatientsObjective:To probe into the influence of weather change on arthralgia of rheumatoid arthritis patients.Method:121 RA outpatients who came from Xiyuan Hospital of China Academy of Chinese Medical Sciences were recruited in this study.Each Patient was asked to fill out the Jamison questionnaire(Pain and Weather Change Questionnaire) and related data was recorded.Result:The majority of RA patients(81.8%) complained that changes in weather affected their pain.Damp,cold and rainy conditions were considered to influence pain the most.Of these patients,56.6%reported that their pain was serious before weather change.They often complained that the pain significantly increased one day before the advent of weather change.Many patients believed that changes in weather had affected the joints most.During weather changes,RA patients felt the knee joints pain the most,followed by the wrist and finger joints.According to the age,≥45 years old and<45 years old,the patients were divided into two groups.Between two groups,joint pain(P<0.01) and duration of morning stiffness(P<0.05) of the former group were more serious than the latter group.The difference was significant.Conclusion:The joint pain and duration of morning stiffness are closely related to the weather changes in the patients with RA,and the sensitivity to weather changes enhanced with the growth of age.Part Three:Effects of Climatic Factor in "Six Seasons" on aggravation of inpatients with RA.Objective:To analyze the effects of seasonal climatic factors on the aggravation of RA inpatients in Beijing area,then reveal the scientific content of the theory of adaptation of human body to natural environment.Method:We have studied 271 RA inpatients came from Xiyuan Hospital of China Academy of Chinese Medicine Sciences during 2000~2006.The meteorological data in Beijing area during these years were obtained from Meteorological Information Center of the China Meteorological Administration.The relationship between RA morbidity and season alternation,or average barometric pressure,or air temperature,or relative humidity,or wind velocity,or precipitation was investigated.Result:1.Difference between the "six seasons"We analyzed the numbers of RA inpatients in 2000~2006,then found that the numbers were different between the "six seasons"(P<0.05).Its peak was appeared in "warm season" and "damp season",and the valley was appeared in "windy season".The difference was very significant(P<0.01).2.Comparison of climatic factors between "warm season" and "windy season" Descending of barometric pressure(P<0.01),enhancing of wind velocity (P<0.05) and precipitation(P<0.01) might be partly of the reasons which made inpatient increase in "warm season".The relative humidity was no different between "windy season" and "warm season"(P>0.05).Relative humidity was similar in "warm season" and "Windy season",and it might not be the reason which made inpatient increase.3.Comparison of climatic factors between "damp season" and "windy season"Descending of barometric pressure and wind velocity(P<0.01),increasing of relative humidity and precipitation(P<0.01) might be the chief reasons which made inpatient increase in "damp season".4.Influence of temperature daily variation on joint pain in RA patientsAccording to the temperature variation,≥3℃and<3℃,the patients were divided into two groups.The joint pain between two groups had a significant difference(P<0.05).The temperature changed more dramatically,the joint pain might be more serious.Conclusion:The seasonal distribution of RA inpatient was due to the change of the climatic factors in "six seasons".The climatic factors,especially their severe fluctuation,could be taken as very important trigger factors of RA.RA has a close relationship with the changes of climatic factor in "six seasons",which coordinates with the theory of adaptation of human body to natural environment.Therefore,the prevention of RA should be strengthened during "warm season" and "damp season".
