Objective:To explore neural function rehabilitation and change of Transcranial Doppler parameters with needling cranial sutures combined with drugs for treatment of acute Posterior Circulation Infarction.Method:According to the clinical diagnostic criterion of the cerebrovascular disease in the fourth national academic discussion of cerebrovascular disease in Chengdu,with the Oxfordshire Community Stroke(OCSP) type of posterior circulation infarction(POCI) diagnosis,40 patients,ranging from 40 to 75 years in age,were enrolled,of which 24 male and female 16 cases.They were divided into medication group(Group A) 20 cases and cranial sutures acupuncture combined with medication group(Group B) 20 cases at random. In Group A,20 cases were treated with a neurology conventional treatment,including modulation arteriotony,prophylaxis hydrocephalus,improve brain circulation,etc;In Group B,besides the same treatment measures as Group A,needling cranial sutures was performed from the Second day of admission to hospital,once daily,for 14 times as a course of treatment.Both Group A and Group B were from first to sixteenth days for observation, compared with neurological impairment score,TCD parameters of change and the relationship between them.Results:(1)Compared with normal level,blood flow velocity decreased,the vertebral arteries blood flow asymmetry more apparent.(2) After treatment,there was a significant decrease of neurological impairment score in both Group A and Group B(P<0.01,P<0.01),but neurological impairment score in Group B changed much more significantly than that in Group A(P<0.05).(3) After treatment,in Group A vertebral artery blood flow velocity,the difference was significant(P<0.05),very different from the basilar artery significantly(P<0.01);in Group B after treatment artery blood flow velocity,the differences were significantly(P<0.01).Between the two groups,artery blood flow velocity differences were not significant(P>0.05).(4)After treatment of vertebral artery blood flow margin compared to group B there was a significant difference(P<0.01),while group A vertebral artery blood flow increased symmetry,but the difference was not significant,not statistically significant (P>0.05).(5) Before and after the treatment of neurological impairment scores and right vertebral artery peak systolic velocity showed low negative correlation(-0.40<R<0,P<0.01),and left vertebral artery peak systolic velocity and basilar artery peak systolic velocity were moderate negative correlation(-0.07<R<-0.40,P<0.01).Conclusion:(1) for patients with acute posterior circulation infarction marked hypo perfusion state,the most important is the performance of blood flow velocity decreased significantly.(2) Transcranial Doppler parameters to reflect the severity level,that is,the lower the blood flow velocity,the more serious neurological impairment.(3) Both medication and cranial sutures plus medication for treatment of acute posterior circulation infarction can improve neural function,but clinical therapeutic efficacy of the latter is significantly superior to that of the former.(4) Needling cranial sutures can accelerate promote the descent of cerebrovascular blood flow velocity and improve neural function in the patients of the acute stage of posterior circulation infarction.
