Observing The Bacteria Of The Uncork With Trocheoctomy Dynamicly After Laryngectomy Or Hypophygectomy

Background and Objective:The infection of the uncork after tracheotomy is one of the most important complications for the local and pulmonary infection.In order to defend and guide the microbial rationalization,induce the infection of uncork and accumulate the clinical data,the dynamic complexions of bacterium in the local trachea were observed afer larygectomy or hypophygectomy and the reason of infection of bacterium.Materials and Methods:32 patients who were suffered with laryngeal carcinoma or hapophrygeal carcinoma and operationed in our department from Mar.2007 to Nov.2007. Thereinto,24 patients were suffered with laryngeal carcinoma,they are all male,the average is 62.6Y;while 8 patients were suffered with hapophrygeal carcinoma,they are all male,the average is 57Y.Bacteria isolated and bacterial susceptibility were performed with the secretion from the uncork after surgery of the 32 patients at the intraday,on the 7th day and the 14th day after the opration.Bacteria isolated were performed with the ATB expression while the Bacterial susceptibility test were identified by th Kirby-Bauer method.Results and Discuss:90 bacterium were cultivated in our investigation from the 32 patients who were performed with laryngectomy and hypophrygectomy on intraday,the 7^th day and the 14^th day.36 Pseudomonas Aeruginosa were cultivated while 18 Methicillin Resistant Staphylococcus Aurues(MRSA) were cultivated in our investigation.But no wancomysin and teicoplanin resistant.At the same time,other serviceable aurues were cultivated,such as Methicillin Resistant Coagulase-Negative Staphylococci(MRCNS) and Klebsiella Pneumoniae which could engenderβ-lactamase.It is a draconic problem about the appearance of Drug-resistance bacteria and it should be regarded adequately.Conclusion:①pathogenic bacteria were cultivated on the uncork on the 7^th day and the 14^th day after laryngectomy or hypophrygectomy;②The bacteria of the uncork after the trocheoctomy is resistant to the antibiotics in the clinical work.③The number of Methicillin Resistant Staphylococcus Aurues(MRSA),Methicillin Resistant Coagulase-Negative Staphylococci (MRCNS),Klebsiella Pneumoniae which could engenderβ-lactamase one the 14^th day is more than the 7^th day.④It is useful for us to know the dynamicly changing and drug resistance of the bacteria on the uncork after trocheoctomy and guide the reasonable using of the antibiotics with observing the bacteria of the uncork after trocheoctomy dynamicly who were performed with larygectomy or hapophygectomy.
