[Objective] To analyze the relation between the improvement rate of the intraoperative DML of the median nerve which compared with the preoperative DML and the prognosis of the carpal tunnel syndrome after surgery.[Material and Methods] 36 patients (40 wrists) who had clinically and electro-physiologically been confirmed carpal tunnel syndrome and had the carpal tunnel release in 2005-2006 are involved in the research. Then the data of all of the cases were recorded. The data included the DML, DMA from the wrist to the abductor pollicis brevis of the median nerve (preoperative, intraoperative and postoperative); the latencies, amplitudes and the conduction speeds of the SNAP from the long finger to the wrist; the Levine carpal tunnel syndrome questionnaire; and the hand function evaluation.[Results] The average score between before the operation and after the operation on the Levine carpal tunnel syndrome questionnaire was improved. All patients can recover a good hand function or above according to the standard. The improvement rate of the postoperative DML of the median nerve which compared with the preoperative DML and which compared with the intraoperative DML has the positive correlation with the improvement rate of the intraoperative DML of the median nerve which compared with the preoperative DML. The improvement rate of the postoperative DML of the median nerve which compared with the preoperative DMA and which compared with the intraoperative DMA has the positive correlation with the improvement rate of the intraoperative DMA of the median nerve which compared with the preoperative DMA. [Conclusions ] People with higher improvement rate of the intraoperative DML of the median nerve which compared with the preoperative DMA can get a better recovery. So do people with higher improvement rate of the intraoperative DMA of the median nerve which compared with the preoperative DMA But to evaluate the prognosis of the patient, other factors include the patient's practical situation should be considered.
