Posterior Cruciate Ligament(PCL)is a main stable structure that retains the flexion and extending and swiveling of the knee joint.,equivalently being as the axle of swiveling action of the knee joint.,its injure results in not only the unsteadiness of recti-direction of the arthrosis,but also the unsteadiness of rotation and latero-direction of the arthrosis,even the degeneration of joint from the long-term point of view,appropriate treatment must be done clinically. The research indicates that the ineffective rate is 20%-25% if the operation has not been done,so we usually do the operations in order to treat it. The force makes the knee joint. excessively- extend injure, which will lead to the injure of PCL. The avulsion injure of PCL, which is located at the attachment region of the tibia is very common. The reports about its treatment and curative effect are very detailed. But The reports about the treatment and curative effect of the avulsion injure of PCL, which is located at the attachment region of the femoral bone is very less .Our hospital has 15 cases ,which consistent with the condition above. All of them are repaired by steel-wire traction through drilling on the attachment region of the femoral bone, which get better curative effect.The cases in the study are all repaired by steel-wire traction ,through drilling on the attachment region of the femoral bone. The curative effect are got by the follow-up visit of the post- operation.Among 15 patients with knee joint PCL avulsion at the end of the femoral bone, 12 patients were male and 3 patients were female, ranging in age from 15 to 75 years old . The average age is 39 years old. 11 cases were traffic accident injury, 3 cases were athletics injury.1 case was Mechanical injury. The position of the injury: 8 cases were left . 7 cases were right.4 cases were old injure,the others were fresh injure.13 cases with other ligaments injured of the same knee. 7 cases with the fracture of lower limb of the same knee.Anteriomedialis approach was adopted and arc incision was made at the wall of knee joint to expose the point of PCL avulsion at the the femoral bone. Repaired the PCL by steel-wire traction through drilling on the attachment region of the femoral bone, Antibiotics was used for 3 to 5 days after operation, as well as plaster external fixation was used for protect the knee. The functional exercise were practiced regularly in all patients step by step according the different stages of recovery.The curative effect are got by the follow-up visit of the post- operation. Lachman and posterior draw tests were negative, and the knee function was better after operation in all cases. According to Lysholm knee joint scoreing system, preoperative and postoperative score were 57.13±6.20 and 88.93±6.20 respectively (t =19.734, P < 0.01). The differences had statistical significance. The function of knee was got better between the preoperative and postoperative.The Conclusion are got from the study.(1) The approach to repair the PCL injure by steel-wire traction ,through drilling on the attachment region of the femoral bone, which the dissection of knee can be exposed clearly. The steel-wire is cheap and can be used in each kind of fracture. The steel-wire can be dislodged from the anterior aspect of the skin , avoiding the second injure of the soft tissue .So ,the approach is the better choice .It is very significance in the primary hospital .(2) The choice of the tunnel of the femoral bone ,which must be in the end of ALB or PMB function bundle.(3) plaster external fixation for protecting the knee must be removed timely. The functional exercise were practiced regularly in all patients step by step according the different stages of recovery. Both of them will make the knee better.(4) The correct diagnose and treatments about the combined injury are helpful to the functional recovery of the knee.
