| Objective:To evaluate the value of parathyroid hormone(PTH),bone specific alkaline phosphatase(BAP),25(OH)vitaminD[25(OH)D]in the diagnosis of renal osteodystrophy among hemodialysis patients.Methods:69 cases of maintanance hemodialysis patients who have already received the treatment more than one year and haven't received pulse therapy of vitamin D(Vit D) were admitted in this clinical trial.Cross-section analysis of intact PTH(iPTH),whole PTH(wPTH),BAP,serum calcium were performed,then CAP/CIP (wPTH/iPTH-wPTH) was calculated.At the same time,48 cases of non-hemodialysis patient were choosed to detect the serum level of 25(OH)vitamin D.Results:69 patients including 36 males and 33 females,average age was 59.4±12.3 years,average dialysis treatment period was 41.7±51.3 monthes,the level of iPTH was 438.26±471.52 pg/ml,there were 32 cases(46.4%of all patients) whose iPTH level were≥300 pg/ml,23 cases(33.3%of all patients) whose iPTH≤150 pg/ml.The level of wPTH was229.16±278.83 pg/ml,which was about 52±19%of the iPTH level.CAP/CIP ratio wa 1.29±1.24,there were 31cases(about 44.9%)of these patients whose CAP/CIP＞1.0。single corralated analysis showed that iPTH was positive related with wPTH,dialysis time,serum phorphate,BAP;however it had no significant related with age and serum calsium.。wPTH was also positive related with BAP;while age,dialysis time,serum calcium and phosphate didn't have significant related with it.25(OH)D was 41.23±16.41nmol/L,97.9%patients among these were lower than 75ng/ml,albumin was the only correlated factor of it.while others like age,iPTH,BAP,serum calcium were not correlated with it.Conclusion:iPTH is close correlated with wPTH and BAP,the accuracy for the prodiction of high turn-over osteodystrophy is 46.4%。CAP/CIP is not related with iPTH and BAP,there is no benification in comparing with wPTH,the meaning of it needs to be elucidated,the level of 25(OH)D is rather lower in most hemodialysis patients,it has no significant correlation with iPTH and BAP.
