Objective:To study the expression of GPC3,CD10,CD34,AFP and HEP in HCC,DN,LCN,FNH and HA,and to explore the utility in differential diagnosis of those hepatic tumors and the value of clinical application.Methods:The expression of GPC3,CD10,CD34,AFP and HEP were detected by immunohistochemistry in 80 cases of HCC(30 cases of W-HCC,50 cases of A-HCC),30 cases of DN(18 cases of H-DN,12 cases of L-DN),36 cases of LCN;80 cases of HCC with pericarcinomous tissues,20 cases of FNH,20 cases of HA.Results:1.The positive expression rate of GPC3 was 92%(46/50) in A-HCC,66.7% (20/30) in W-HCC,11%(2/18) in H-DN,no expression in L-DN,LCN,FNH and HA.The positive expression rate of GPC3 in W-HCC was lower than in A-HCC,the different expression had statistical significance,P<0.05.This indicated that the expression of GPC3 was correlated with tumor differentiation and was higher in poor differentiation tumor;The positive expression rate of GPC3 in W-HCC was higher than in H-DN,L-DN,LCN,FNH,HA(P<0.05).2.The positive expression rate of AFP was 44%(22/50) in A-HCC,20%(6/30) in W-HCC,no expression in DN,LCN,LCN,FNH and HA.The positive expression rate of AFP in W-HCC was lower than that in A-HCC,The different expression had statistical significance,P<0.05.This indicated that the expression of AFP was correlated with tumor differentiation and was higher in poor differentiation tumor; The positive expression rate of AFP in W-HCC was higher than that in LCN,FNH and HA.The different expression had statistical significance(P<0.05),but no difference with H-DN and L-DN. 3.The positive expression rate of HEP was 72%(36/50) in A-HCC,90%(27/30) in W-HCC,100%in H-DN,L-DN,LCN,FNH and HA.The positive expression rate of HEP in W-HCC was higher than that in A-HCC,The different expression had statistical significance,P<0.05.This indicated that the expression of HEP was correlated with tumor differentiation and was lower in poor differentiation tumor;no significantly difference in W-HCC compare with H-DN,L-DN,LCN,FNH and HA.4.The negative expression rate of CD10 was 78%(39/50) in A-HCC,43.3% (13/30) in W-HCC,20%(4/20) both in FNH and HA;the positive expression cells>50%(+++) was 2%(1/50) in A-HCC,16.7%(5/30) in W-HCC,But the positive expression cells>50%(+++)was 83.3%(15/18) in H-DN,91.7%(11/12) in L-DN,80.6%(29/36) in LCN,60%(12/20) both in FNH and HA.The positive expression rate of CD10 in W-HCC was higher than that in A-HCC,The different expression had statistical significance,P<0.05.This indicated that the expression of CD 10 was correlation with tumor differentiation and was lower in poor differentiation tumor;The positive expression cells rate of CD10 in W-HCC was lower than that in H-DN,L-DN,LCN,FNH,HA(P<0.05).5.The percentage of positive cells of CD34 in A-HCC and W-HCC ranged from 25%to 100%,"+++"was 76.0%(38/50) and 70.0%(21/30);however in H-DN and L-DN groups,it ranged from 5%to 25%,"+" was 88.9%(16/18) and 83.3%(10/12),in LCN group,ranged from 0%to 5%,"+" was 27.8%(10/36).In FNH and HA groups,it ranged from 25%-50%.The percentage of positive cells of CD34 in W-HCC was significantly higher than that in H-DN,L-DN,LCN,FNH and HA (p<0.05),but no significantly difference compared with A-HCC.6.The detaction rate for HCC with GPC3,CD10,CD34 and AFP was 82.5%,65%,60%,47.5%respectively.The detaction rate for HCC with the combined markers:GPC3/AFP,GPC3/CD10,GPC3/CD34,GPC3/CD10/AFPwas 87.5%,93.75%,95%and 95%respectively.The detaction rate for HCC with single marker comparing with that of correspond combined markers was significantly different,excepet for GPC3 compared with GPC3/AFP.7.The expression of GPC3,CD10 and CD34 had no prognosis value in HCC(P>0.05).The positive expression of AFP had poor prognosis(P<0.05),but the positive expression of HEP had well prognosis(P<0.05).Conclusions:GPC3 is a very sensitive and specific marker in diagnosis and differential diagnosis on HCC,combined with AFP,CD34 and CD10 is useful in differentiating HCC from DN,LCN,FNH and HA;The high expression of AFP and the low expression of HEP could be used as feasible determinants for bad prognosis of HCC.
