Objective:To prospectively investigate the diagnostic value of MRI and sonography in breast tumors.Methods:Forty cases with breast tumors were performed with MRI routine imaging,diffusion weighted imaging(DWI),dynamic enhanced-MRI(DCE-MRI) and sonography.Among these cases,30 cases were performed with whole body diffusion imaging(WB-DWI).(1)Routine imaging:observe breast Wolfe typing,lesions' location,shape,border and signal etc.(2) DWI: analyze lesions' signal and measure their apparent diffusion coefficient(ADC),signal intensity and relative ADC to the opposite side mammary gland(rADC1).Analyze breast lesions' signal in WB-DWI and measure their ADC,signal intensity and relative ADC to the spinal cord in the same layer(rADC2).(3)DCE-MRI:analyze lesions' enhanced mode and invasion.Draw time-signal intensity curves(TIC).Record the peak time,the maximum signal intensity and compute several parameters,such as the maximum enhancement ratio.(4)Sonography:analyze lesions' boundary,internal echo,posterior echo and measure the ratio of lesions' depth to width in the bidimensional grey-scale sonogram,record blood flow Adler grade of breast lesions and measure peak systole velocity(PSV),end diastolic velocity(EDV) and resistance index(RI) in CDFI.All statistics were carried out through SPSS 13.0.Results:There were 30 benign and 28 malignant lesions.(1)Type P2 was predominant in the malignant lesions(43.5%),while type DY was predominant in the benign lesions(41.2%). Lesions' location,shape and border had differential diagnostic value.(2)The detection ratio of DWI was 100%.The ADC threshold of benign and malignant lesions was 1.688×10-3mm2/s, which diagnostic accuracy was 74.3%.The rADC2 threshold was 36.7%,which diagnostic accuracy was 74.1%.(3)DCE-MRI could detect multi-center lesions and invasion.The peak time threshold of benign and malignant lesions was 6m20s,whic.h,diagnostic accuracy was 62.9%. (4)The Adler grade could differentiate malignant from benign lesions,gradeⅡandⅢwere predominant in malignant lesions.(5)The diagnostic accuracy of MRI multi-sequences imaging was 98.3%.The combination of MRI and sonography was 100%.Conclusions:(1)DWI has high detection ratio of breast tumors.The ADC and rADC2 have high diagnostic value.(2)DCE-MRI can detect multi-center lesions and infiltration to the adjacent tissue.It is valuable for preoperative evaluation.The TIC type can differentiate malignant tumors from benign tumors.(3)MRI combining sonography raises the diagnostic accuracy of breast tumors.
