Objective:1.Decompose New Zealand albino rabbits' single segment of lumbar vertebra stable structure partly leading to its early lumbar intervertebral disc degeneration model and observe the rabbits' lumbar intervertebral disc changes of magnetic resonance imaging(MRI) and pathology,compare the difference from disparity phases and research the changes of early lumbar intervertebral disc degeneration in pathology and iconography.2.Treat degenerative intervertebral disc of these model mentioned above in vivo by injecting the gene of human insulin-like growth factor-1 mediated adenovirus(Ad- hIGF1) and human insulin-like growth factor-1(hIGF1) individually,compare and observe the different changes in magnetic resonance imaging(MRI) and pathology.Then provide experimental evidence for early prevention and treatment of lumbar degenerative disease.Methods:1.60 healthy New Zealand rabbits are selected for the test.60 rabbits are divided into 2 groups which are the test group and the control group,the test group has three 30 rabbits and the control group has 30 rabbits selected in random ways.Operating on group of test rabbits,decompose supraspinal ligament, interspinous ligament and lumbar myo- stops,group of control rabbits only have an operation with incision of skin and sew up.Breed these rabbits under the same condition,do MRI test 2months,4months and 6months respectively after surgery.The tissue of lumbar vertebrae was harvested from both groups in 2 months,4 months and 6 months respectively after operation for the test of pathology,then compare the difference of pathological change between these 2 groups.2.Choosing 32 healthy New Zealand rabbits with 6 month.Operating on rabbits,leading to lumbar vertebra intervertebral disc degeneration.Dividing these rabbits into 3 groups selected in random ways which are the transgene group -16 rabbits,the cytokine group - 8 rabbits and the control group - 8 rabbits.Raise these animals under the same condition,then inject the Ad-hIGF1 into degenerative intervertebral disc of the transgene group, hIGF1 into the cytokine group,and the isotonic Na chloride injection into the control group.Harvest the tissue of intervertebral disc injected into healing factor in 2weeks, 1month and 2months respectively after injection in order to observe the expression of human insulin-like growth factor-1,compare these 3 groups with MRI test and pathology in 2months after injection.Results:1.MRI test showed that there is no difference between the test group and the control group after 2 months later from the time of destabilization operation.The lumbar intervertebral disc on range of operation which is test group appear hypo-signal through MRI T2WI test in 4 month,6 month after surgery,the T2WI signal intensity of lumbar intervertebral disc in 4months after surgery are higher than the signal intensity after 6 months which some of turn black.There is no spinal deformity on the experiment,such as lateral curvature,aboral prominence and intervertebral space,etc.In the appearance of HE staining of pathology,there is some of alteration on the ground substance of end plate between the test group and the contro1 group after 2 months later from the time of destabilization operation,after that, the disc cell of the test group are not regulation compared to the cell of the control group,endplate cell layers obviously decreased,the thickness of endplate getting thin, cell degeneration appeared there and the area of blood vessel under the endplate reduced.The change mentioned above after 6 month are more serious than the change after 4 months,and there is no clear demarcation between the cells in nucleus pulposus,which degen cells appeared,the number of cell decreased,the size of cell turned smaller.The consequence of gray scale value of PAS stain measured by computer pathology system shown that the control groups' gray scale value is the lowest,the lower one is the test group in 2 months after surgery,and the gray scale value are getting higher on the test group in 4 months after operation,the value is the highest on the test group when the time is to 6 month after lumbar vertebra were destabilizing.2.The result analysis of the tissue applied by RT-PCR technique analysis was harvested,it showed the expression of hIGF1 at intervertebral disc obviously presented in transgene group.The strongest expression is in 2weeks, gradually followed by the expression in 1 month and in 2months,while the expression of hIGF1 is not present in cytokine group.3.After 2 month later from the time of injection,the lumbar intervertebral disc on range of operation showed hypo-signal through MRI T2WI test on the transgene group,the cytokine group and the control group,and the T2WI signal intensity of the transgene group are higher than the cytokine group and thecontrol group.MRI test showed that there is no obviously distinction between the cytokine group and thecontrol group.The appearance of HE staining of pathology shown that,the nucleus pulposus cells of both two control groups are not regulation compare to the transgene group,which degen cells appeared, the number of cell decreased,the size of cell tumed smaller,and here is no clear demarcation between the cells in nucleus pulposus,but no obviously difference was about endplates.There is no distinction on the gray scale value of PAS among the transgene group,the cytokine group and the control group.Conclusion:1.Decomposing supraspinal ligament,interspinous ligament and lumbar myo- stops can lead to degeneration in lumbar cartilage end plate,vertebral pulp and annulus fibrosus slowly but gradually.2.The gene of human insulin-like growth factor-1 mediated adenovirus have some effects when it use to treat intervertebral disc degeneration.
