1.Analysis Of Relationship Between Insulin Resistance And Dyslipidemia In Type 2 Diabetes Mellitus Associated With Nonalcoholic Fatty Liver 2.The Preliminary Study On Vascular Endothelial Growth Factor Gene Therapy Of Skin Wounds In Diabetic Rats

[Objective]In this study,we observed clinical features and blood biochemical parameters of type 2 diabetes mellitus(T2DM) incorporated with non-alcoholic fatty liver disease(NAFLD) or not,and analized risk factors during the occurrence and development of T2DM with NAFLD,so as to explore the relationship between insulin resistance and abnormal lipid metabolism,which providing a clinical basis for the prevention and treatment work in the end.[Methods]Illness histories in 99 patients of T2DM with NAFLD and 101 patients of T2DM were taken and compared.To check features including height, weight,waist circumference,hip circumference,blood pressure;liver enzymes,blood lipids,fasting and postprandial 2 h blood glucose,insulin,C-peptide levels and HbA1C measurement,body mass index(BMI),waist-hip ratio(WHR) and improved insulin and C-peptide index(HOMA-C-peptide),and contrast to 101 patients with non-alcoholic fatty liver disease(NAFLD).Datas are analized by the SPSS11.5 statistical computer software,statistical results with statistical significance by P<0.05.[Results]:1.Triglyceride(TG),BMI,alanine aminotransferase(ALT)are the major risk factors of the type 2 diabetes mellitus with fatty liver.2.Compared with the control group,the T2DM with NAFLD group has lower age and shorter duration (p<0.05),but there is no difference in the gender composition(P>0.05).3.Weight, body mass index,waist circumference,hip circumference,ALT,aspartate aminotransferase(AST),TG,total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C) were significantly higher in the T2DM with NAFLD group (p<0.01).High-density lipoprotein cholesterol(HDL-C) was lower than the control group(p<0.05).4.Fasting blood glucose(FBG),fasting and 1 hour postprandial, C-peptide,HOMA-C-peptide index were significantly increased in the NAFLD group, and there was a significant difference(p<0.01).Postprandial 1 hour blood glucose, HbA1C were also increased(p<0.05).5.The incidence of fatty liver in the hyperlipidemia was significantly higher than in the non-Hyperlipemia,with a significant difference(p<0.01).The incidence of fatty liver in the simple high TG group with combined hyperlipidemia was significantly higher than the normal group, with significant differences(p<0.01);However,the incidence of fatty liver has no significant difference between the simple high-TC hyperlipemia groups and normal groups(p>0.05).[Conclusion]:1.The higher TG,ALT level,abdominal obesity and insulin resistance are the major risk factors of the T2DM with NAFLD.2.The T2DM with NAFLD patients should use drugs to prevent from liver damaging.Also,liver function should regularly be reviewed in case of liver cirrhosis.3.In the prevention and treatment of T2DM with NAFLD,therapies such as improving insulin resistance, enhancing insulin sensitivity are really necessary.At the same time,strict control of blood glucose,blood lipid levels can reduce and delay the T2DM duration.There have a significant meaning. [Objective]By mergeing the plasmid pMSCV-VEGF165-GFP with the vector PEI-(PEG550)10,and than fix them to the collegen material to get a non-virus vector induced gene expression system which used for the therapy of diabetes rats medels with cutaneous chronic wounds.We compare the recovery of the woulds,new-born blood vessel and the exprssions of hVFGF gene in different groups,then analyze the effect of this mathod to diabetes rats with cutaneous chronic wounds and how it affects the wounds recovery process,meanwhile,collect more data for clinical therapy of this kind of disease.[Methods]To build the diabetic rats medels with cutaneous chronic wounds. Four groups are determined according to blood-sugar content:Normal-control groups (groups with normal blood glucose);DM-blank groups(groups with non-theropy); DM-Gel group(groups with therapy of collegen+PEG/PEI vector);DM-DNA groups (groups with therapy of pMSCV-VEGF165-GFP plasmid which contains DNA+gel+ PEG/PEI vector).After the establishment of diabetes rats medels with cutaneous chronic wounds,we treat different methods with different groups as written before,and choose 4 rats from every groups randomly 5 and 14 days after the treatment,execute them and cut the granulation tissues,8 woulds for every group.Data that needed to be analized:1 the rates of the recovery of woulds.2 vwf-immunostaining to examine the states of new-born blood vessels.3 the expressions of important genes by RT-PCR[Results]1.Conditions of the recovery of woulds:DM-DNA groups and the Non-DM groups have the same recovery rate,which is obviously better than the rate of DM-Control groups;DM-gel groups have the better rate than DM-Control groups,but not that better than DM-DNA groups.2.DM-DNA groups have more new-born blood vessels than other groups.3.In DM-DNA groups,we find obvious exprssions of h-VEGF gene.[Conclusion]When it treated to the diabetes rats medels with cutaneous chronic wounds,The mix of plasmid pMSCV-VEGF165-GFP transfected by PEI-(PEG550)10 vector with collegen,can advance the expression of the gene VEGF of the woulds,and shorten the growing time of the granulation tissues,promote the growing of new vessels and woulds healing.
