| Epilepsy is a group of brain disease due to excessive discharge by neuron, which leads to recurrent,sudden and short-term central nervous system dysfunction.It's common in neurology and belohgs to "epilepsy" in Chinese medicine.At present,the pathogenesis of this disease is not yet completely clear,clinical manifestations are complicated,course is long,treatment is not easy.According to epidemiological data,epilepsy incidence in people is about 50～70/(·100,000),annual prevalence rate is about 5%,according to this rate,there is approximately 65 to 70 million new patients with epilepsy each year,there are more than six million patients with epilepsy.Modern medicine for the understanding of epilepsy has been greatly developed,but now,the pathogenesis of this disease has not yet completely clear,the treatment is also very slow progressed.Both the traditional antiepileptic drugs,like conventional medicine such as sodium valproate,carbamazepine, or new developed antiepileptic drug,such as ammonia own acid,lamotrigine may have an adverse reaction,the severity varies depend on different individuals,and these existing anti-epileptic drugs can not completely control all epileptic attack,they also have a variety of adverse reactions, leads to the decline of patients' life quality,many patients can not endure long-term drug treatment,so epilepsy is not only the medical problems but also the public and social issues,WHO has labeled epilepsy as an important nerve and mental disease that needs prevention and treatment. With the overall concept and syndrome differentiation,Chinese medicine shows a unique advantages and clinical vitality in complex diseases.Chinese medicine believes that pathogenesis of this disease is complex,in general, it refers to liver,kidney,spleen deficiency as disease-oriented basis,As to pathogenesis,it always due to wind,fire,phlegm,blood stasis and deficiency,as to the treatment,it always prefer the way of calming down the wind,cleaning the fire,eliminating the phlegm,expelling the blood stasis. Chinese medicine has advantages in treatment of epilepsy,first of all, Chinese medicine in epilepsy reflects advantage of overall treatment.Second, Chinese mediciine has obvious advantage than Western medicine in treatment of remission period.Third,Chinese medicine has little side effects.Clinical application of Chinese medicine treatment of epilepsy was rewarded with satisfied results.Therefore,using the diagnostic methods of traditional Chinese medicine,combining the four diagnostic way,and grasping the consistency of rationale,method,decoction,herb,to get better effect,make Chinese medicine in the treatment of epilepsy reached a higher level. "Tian Xing Ding Xian Capsule" is one kind of new drug for epilepsy developed by Affiliated Hospital of Henan Traditional Chinese Medicine Research Institution,it contains more than 20 components,such as artificial bezoar, saiga horn,pearls,Polygala tenuifolia,Acorus gramineus,all insects, centipede,free oneself,ginseng,gastrodin,Jiangcan,gall Southern Star, Tenjiku yellow,amber,salvia,the function are as follows:expelling phlegm and cleaning heat,ventilating lung and moving wind,calming down wind and releasing spasm.This capsule is for epilepsy of syndrome of phlegm and fire Closing,liver wind rebelling.This research was to study the Clinical efficacy of the treatment of epilepsy,through literature,different syndrome and clinical observation of "Tian Xing Ding Xian Capsule",and explore the effectiveness of using Chinese medicine" Tian Xing Ding Xian Capsule" as a complementary antiepileptic drug together with the first-line drugs(sodium valproate) to treat epilepsy.This research adopted 30 cases of epilepsy patients of standard selection criteria for clinical observation analysis,preliminary observed the possible mechanism of" Tian Xing Ding Xian Capsule" in treating epilepsy,in order to provide a guide in using Chinese medicine as a clinical treatment of epilepsy. The theoretical part organized studies of the theoretical basis of epilepsy and the treating rule,based on the collection of documents.(1) summarizes the origins of epilepsy treatment,pathogenesis,and the rule of treatment,and so on.(2) The theory of "Treatise on Febrile Diseases" for epilepsy,explores and understands the epilepsy Treatment based on the six meridians,The book did not set up a special article on epilepsy,but there are many similar descriptions of the symptoms of epilepsy,can also guide the clinical treatment of epilepsy.(3) finishing the treatment experiences for epilepsy of my tutor, analysis and explore the relationship between Ying-Wei Theory,Spleen-Stomach Theory and Shen-Brain Theory In the Part of the clinical observation,discussing the clinical efficacy and safety evaluations of" Tian Xing Ding Xian Capsule",as a supplementary medicine,in the treatment of epilepsy of phlegm-fire closing and liver-wind rebelling.(1) research program is designed like this:in out-patient department, to label 30 patients as research objection according to the selection criteria, to make clinical observation for epilepsy treatment efficacy of phlegm-fire closing and liver-wind rebelling.(2) Analysis "Record of drug and clinical trial reports of Tian Xing Ding Xian capsule".Ask each patient to submit every month when they come back, for six months in total.(3) compare the results of EEG before and after the treatment,and evaluate the efficacy and safety of "Tian Xing Ding Xian capsule "in clinic. Professor Zhang Hengliu assembled research and clinical experience of "Treatise on Febrile Diseases" for four decades,He suggested that the pathogenesis of epilepsy,which is because of spleen deficiency,obstruction of wind and fire and phlegm with blood stasis,resulting in,first,disharmony of Ying-Wei,unsmooth of Qi,imbalance of organ-Qi,unconnected of Ying and Yang,second,fundamental deficiency and appearance excess,He believes the fundament of pathogenesis in epilepsy is spleen deficiency,and the appearance of pathogenesis is wind-phlegm with blood stasis,the epilepsy patients are mostly because of gallbladder depress and spleen deficiency,so he also suggests treatment should be to ventilate the Qi,balance the Ying-Wei, reinforce the Spleen-Qi,calm down the wind,expell the phlegm and move the blood and stasis.Professor Zhang picked and modified the method of ventilating the Qi and balancing the Ying-Wei from "Treatise on Febrile Diseases",created "Xian Ning Pill",from theory to clinical and pharmacological mechanisms, it has achieved a satisfactory treatment effect from epilepsy patients of gallbladder depress and spleen deficiency.This research,through theory research and clinical observation of both sides,systematically summarizes the 30 cases of epilepsy patients s in line with selection criteria,and observes the clinical efficacy of "Tian Xing Ding Xian Capsule" to the epilepsy of phlegm-fire closing and liver-wind rebelling.So it can play a further guide for Chinese Medicine in clinic of treatment of epilepsy. |
