Objective:To explore the characteristics and to evaluate the diagnostic valuation of DSA at PACNS.Methods:22 cases of PACNS diagnosed by combining the clinical materials and DSA findings in the First Affiliated Hospital of Shanxi Medical University from January 2002 to december 2008 were enrolled into the study. We reviewed their DSA findings and analyzed the morphologic changes and distribution of involved vessels systematically. Results:There is 41 vessels involved in the 22 cases.Their morphological changes are diversity and can be classified as 3 types:stenosis 28,occlussion 11 and aneurysm/tumoroid change 2.Among them stenosis include:single local stenosis(9),multilocal stenosis(7) stenosis, segmental stenosis(8),beaded changes(4),occlussion include:5 with tributaries conformation,6 no tributaries conformation. Conclusion:The clinical presentation of PACNS is not special.Morphological changes of involved vessels is diversity.Mainly is variant stenosises and occlusions, the lumen of involved vessel is symmetric.Their distribution has a predilection for unilateral cerebral vessels and cerebral vessels of anterior circulation.Cerebral angiography is an important and feasible examination for the diagnosis or exclusion of PACNS at present. It is a safe, minimally invasive, high resolution, less complication technique and easily accepted by patients. Moreover,it is the basis and premise for targeted biopsy. It can direct physicians to make a right and prompt treatment for patients and avoid unwanted complications due to drug abuse.
