| Objective: To evaluate the Application value of CT scan in diagnosis of distal tibiofibular syndesmosis injury,and provide an effective method for clinlic early diagnosis.Methods: 68 patients with closed ankle fractures indicated for operative stabilization were treated by open reduction and internal fixation. The tibiofibular syndesmosis were intraoperative conventional explored , and the exploration results were used as gold standard to evaluate the diagnostic values of radiography and CT scan examinations in this present study. All patients were taken both radiography and CT scan examinations preoperatively, and the situation of distal tibiofibular syndesmosis were evaluated by two doctors independently. The authors comparing the evaluation results of X-ray and CT scan respectively with surgical exploration, then calculating the respective evaluation of diagnostic tests and analyzing the parameters statistically.Results: 68 cases of ankle fracture patients were hospitalized and treated by operation, in which 46 cases were diagnosed as distal tibiofibular syndesmosis injury, 22 cases diagnosed as without distal tibiofibular syndesmosis injury after explored the distal tibiofibular syndesmosis. X-ray plain film diagnosed as true positive in 25 cases , true negative in 16 cases, false positive in 6 cases, false negative in 21 cases . CT scan diagnosed as true positive in 43 cases , true negative in 21 cases, false positive in 1 cases, false negative in 3 cases . In Comparison with surgical exploration results, the sensitivity, specificity,and accuracy were 54.34%, 72.72% and 60.29% for radiography and 93.47%, 95.45% and 94.11% for CT scan in diagnosis of distal tibiofibular syndesmosis injury,and there were statistically differences between two methods.(P value< 0.01).Conclusion: conventional radiography is not reliable in assessing distal tibiofibular syndesmosis injury,but CT scan is very reliable with good validity and can provide an important reference value. |
