Objective: To compare the differences of three diagnostic criteria for metabolic syndrome (MS), issued by the National Cholesterol Education Program (NECP), International Diabetes Federation (IDF) and Chinese Diabetes Association (CDS).Methods: The prevalence of MS in people underwent health care screening in our hospital from April 2006 to June 2006 was calculated and analyzed according to three definitions. Among 9066 screened persons, 3099 subjects with complete data, including background information of each individual, blood pressure, lipid profile, plasma glucose were analyzed. MS was defined according to three different diagnostic criteria, i.e., ATPâ…¢, IDF, and CDS criteria.Results: (1) The number of MS patients in those screened people diagnosed by at least one criterion was 977, with 639 males and 338 females. The prevalence of MS was 31.5% in overall, with 36.39% in male and 25.17% in female. For ATPâ…¢criterion, 851 were diagnosed as MS, with 547 males and 304 females. The overall prevalence of MS by ATPâ…¢was 27.46%, with 31.15% in male and 22.64% in female. For IDF criterion, 738 were diagnosed as MS, with 447 males and 291 females. The overall prevalence of MS by IDF criteria was 23.81%, with 25.46% in male and 21.67% in female. For CDS criterion, 676 were diagnosed as MS, with 472 males and 204 females. The overall prevalence of MS by CDS criteria was 21.81%, with 26.88% in male and 15.19% in female. ATPâ…¢definition has the highest sensitivity detect MS among the three criteria. (2) Kappa index was 0.905 between ATPâ…¢and IDF criterion, with 0.860 in male and 0.631 in female. Kappa value was 0.643 between ATPâ…¢and CDS criteria, with 0.596 in male and 0.631 in female. At last, Kappa value was 0.593 between IDF and CDS criteria, with 0.595 in male and 0.584 in female. The ATPâ…¢and IDF criterion showed excellent consistency, whereas the CDS criteria showed poor consistency with the other two criteria. The consistent rate was 96.35% between ATPâ…¢and IDF definition, 86.22% between ATPâ…¢and CDS definition, and 85.67% between IDF and CDS definition. (3)According to the above three criteria (ATPâ…¢, IDF and CDS), central obesity was present in 86.96%, 100%, and 89.35%, respectively; hypertension in 72.74%, 69.92%, and 43.79%; hypertriglyceridemia in 66.16%, 61.38%, and 67.01%; low HDL-C in 1.76%, 3.12%, and 0.3%; hyperglycemia in 90.25%, 89.57%, and 61.98%, respectively.Conclusion:The ATPâ…¢and IDF criterion showed good consistency, whereas the CDS criterion has lower consistency with the other two. The ATPâ…¢criterion has the highest sensitivity for diagnosing MS.
