Objective:①Detected the expression of MPO,LPO,SOD and GSH in fetal membranes of normal pregnancies at term and patients with premature rupture of membranes,to evaluate the correlation between the oxidative damage and premature rupture of membranes.②To evaluate the function of oxidation and anti-oxidation in the pathogenesis of premature rupture of membranes.Methods: 1 The study subject:(1) The preterm premature rupture of the membrane group: 20 patients with premature rupture of membranes who had prenatal examinations and delivered in the Obstetrics of the Second Hospital of HeBei Medical University were selected from October 2007 to June 2008. Preterm premature rupture of the membranes (P-PROM) is diagnosed when clinically apparent leakage of amniotic fluid is confirmed in pregnant women without uterine contractions or vaginal bleeding before the 37 weeks' gestation..They were diagnosed according to the diagnostic standard (Obstetrics and Gynecology,the Sixth Edition).Mean age of this group was 28.38±5.01 years old (23 to 37 years old),and mean gestational age was 35±0.75 weeks(34+1 to 36 weeks of gestation).(2)The full term premature rupture of the membrane group:20 patients with premature rupture of the membrane after 37w were selected,who were delivered in our hospital in the same time. Mean age of them was 29.1±3.21 years old (26 to 35 years old),and mean gestational age was 39.2±0.63 weeks(38+6 to 40+3 weeks of gestation).(3)normal pregnancy group:20 normal pregnancy march with the two groups were randomly selected.Mean age of them was 27.67±1.86 years old (25 to 30 years old),and mean gestational age was 39.33±1.21 weeks(38+5 to 41 weeks of gestation).All the subjects must meet the following criteria: single pregnancy, head fetal position, with no complications of intered medicine and other complications of pregnancy.2 Sample collecting: Each specimen(1.5 cm×1.5 cm) close to the defeat place was collected after cesarean or vaginal delivery, and then remove its bloodstain and amniotic fluid on the surface, reserve them to the refrigerator of -70oC. Marked every object with its name, age, weight index, occupation, times of pregnancy, gestational weeks, delivery style, reason of cesarean,time from the membrane rupture to collecting the specimen. Every specimen must be signed clearly.3 The experiment methods: Make the tissues to homogenate after collecting specimen. Then, determine the level of index according to the kit specification. The levels of MPO and GSH were tested by chromatometry. The levels of LPO were tested by Thiobarbituric acid reactive substances(TBARS). To assay the levels of SOD by means of xanthine and xanthine oxidase reaction system.4 Statistics analysis: All the data were processed by SPSS13.0. All values were expressed as mean±standard (?x±s). Deviation and differences were compared using Student's t-test and one-way ANOVA. Pearson correlate was made for correlate analysis.α<0.05 was consideredstatistically significant.Results: 1 The expression of SOD in PROM group was lower than the normal group (the average value: 248.34±22.23 vs 308.43±22.54), The differences were significant in statistics(P<0.05).2 The expression of SOD in full term PROM group was stronger than the P- PROM group (the average value: 254.70±19.44 vs 241.98±23.55 ),There was no statistical differences(P>0.05).3 The expression of LPO in PROM group was stronger than the normal group (the average value: 4.00±1.13 vs 2.46±0.52), The differences were significant in statistics(P<0.05).4 The expression of LPO in full term PROM group was stronger than the P- PROM group (the average value: 4.04±0.97 vs 3.95±1.29 ),There was no statistical differences(P>0.05).5 The expression of GSH in PROM group was lower than the normal group (the average value: 88.52±10.54 vs 123.75±8.98 ), The differences were significant in statistics(P<0.05).6 The expression of GSH in full term PROM group was stronger than the P- PROM group (the average value: 85.16±10.36 vs 91.89±9.84 ),There was no statistical differences(P>0.05).7 The expression of MPO in PROM group was stronger than the normal group (the average value: 0.20±0.08 vs 0.11±0.50 ), The differences were significant in statistics(P<0.05).8 The expression of MPO in term PROM group was stronger than the P- PROM group (the average value: 0.21±0.09 vs 0.20±0.08),There was no statistical differences(P>0.05).9 Pearson correlation analysis: the expression of SOD in normal group and PROM group were negative correlation with LPO(r=-0.438,P<0.01) and MPO(r=-0.393,P<0.01)and positive correlation with GSH(r=0.677,P<0.01).The expression of GSH were negative correlation with LPO(r=-0.525,P<0.01)and MPO(r=-0.513,P<0.01).Conclusion: 1 The expression of MPO and LPO in the PROM group was higher than the normal group. It indicated that the ROS and lipid peroxidation product were increased in women of PROM.2 The expression of SOD and GSH was lower in the PROM group than the normal group. It indicated that the antioxidant defense system was weakened in women of PROM.3 The index had no significant differences in term PROM and preterm PROM group. We can illustrate that the oxidative stress has no relation with gestational age.4 The balance of oxidative system and antioxidant defence system was broken, when the oxidative stress generate, and it play significant role in the PROM.5 With the increase of ROS , the anti-oxidant decreased. The system of anti-oxidation was destroyed, what lead to the oxidative stress. It damage the function of organism , then the membrane destroyed.
