Malignant tumor is one of chronic disease which seriously threat to the quality of human being.Studying on the disease burden of malignant tumor is useful to the hygiene department to make decision,allocate the health resources rationally,and also be a great significance to improve the quality of our life.This paper includes two sections.In the first section the existing data was used to calculate the burden of disease caused by cancer,the economic burden of disease caused by cancer.In the second section the data of the patients with breast cancer was collected by the questionnaires and the main impact factors were explored on the patients' hospital fees.Some suggestions and references for the medical service organization were provided.Partâ… Study on the disease burden of cancer in ChinaThe existing data published by Health Ministry of China is mainly measured with traditional indicators such as 'morbidity' and 'mortality'.However,they could not reflect the disease' impact on people' s social value and also difficult to indicate the degree or the duration caused by the disease although the type of these indicators are simple and easy to obtain.This paper adopts DALY which can reflect both disability and death lost caused by disease.And DALY is also recommended by the World Health Organization(WHO).Combined with the data which published by the Health Ministry to estimates the burden of disease caused by cancer,the economic burden of disease caused by cancer,the conclusions were made as follows:1.The DALY loss caused by cancer in year of 2006 was 5,423,882.16 person-years in which 3,428,318.62 was from male and 1,995,563.54 from female.The loss caused by death(YLLs) and disability(YLDs) were 5283947.99 person- year and 139,934.17 person-year respectively.2.The top five malignant tumors sorted by 'mortality' were:lung cancer,liver cancer,stomach cancer,esophagus cancer,colon cancer. But sorted by DALY,the top five were:lung cancer,stomach cancer,breast cancer,liver cancer and cervical cancer.Based on the DALYs the first order was lung cancer in male and breast cancer in female.3.The DALY loss per thousand in 2006 was 19.69 person-year,higher than 14.94 person-year which were announced by WHO in 2002.It shows that the disease burden caused by cancer had increased.The data is similar to the level of European(19.55) in 2002,but higher than the Global Level (12.14) in 2002.4.Among the economic loss caused by malignant tumors in 2006,the direct economic loss was 70.328 billion yuan and indirect economic loss was 37.268 billion yuan.The total loss was 107.596 billion yuan which was 0.51%of the GDP and 4.67%of the total medical cost of China in 2006.Partâ…¡Study on the treatment costs of breast cancerFrom Partâ… of the analysis,we had known that the economic burden caused by malignant tumor accounted for a large proportion of total health expenditure,this study intended to explore the factors that affect the treatment cost of malignant tumors,so as to develop some practical measures to promote the progress of the health care reform.In this paper, we took breast cancer as an example,to design the questionnaire and collect data in the field of socio-economic characteristics,disease characteristics and treatment information,using non-parametric and multiple stepwise regressions to identify the impact factors of the expense of the patients with breast cancer.It was made the conclusion that:1.The survey collected 173 cases of patients;the median of the hospital charge and treatment times is 21,865.23 yuan and 3 times respectively.The average of hospital charge is 759.97 yuan per day.Among the hospital charge,the costs of medicine accounted for the largest proportion(61.07%),followed by the treatment costs(14.09%),living costs(8.98%),inspection costs(8.52%),operation costs(6.97%).2.Multivariate analysis showed that the factors which influence the hospital costs were as follows:the number of seeing dactor, hospitalization days,and the way of payment,occupation,Lymph node metastasis and type of treatment.While making decisions,the health department should consider these factors to manage the factor of controllable cost so as to reduce the total health expenditure.
