A Preliminary Study On Quality Of Life And Detection Of Endothelin-1 In The Plasma And Skin Tissue Fluid On Patients With Vitiligo

Objective:1.To evaluate the effect of vitiligo on quality of life in Chinese patients.2.To study the role of endothelin-1(ET-1) in the pathogenesis and prognosis of vitiligo,level of ET-1 in the plasma and skin tissue fluid of patients with vitiligo was detected.Materials and Methods:1.We collected eligible data from 1025 patients with vitiligo by a uniform questionnaire about clinical and epidermiology study,of which 786 patients were evaluated by the measurement of quality of life.And we collected samples of plasma (n=96) and lesion tissue fluid(n=25) from vitiligo patients.30 healthy people and 25 normal skin tissue fluid were set as controls.2.We evaluated the effect of vitiligo on quality of life in Chinese patients by DLQI.3.We measured the levels of ET-1 in the plasma and the tissue fluid from vitiliginous lesion and the normal skin by the method of radio immunity.4.Statistical analysis was performed by one-way ANOVA and LSD-test.Results:1.The overall mean DLQI score for vitiligo was 7.32±5.68,which meant vitiligo had moderate effect on patient's life.The individual proportions of five grades were:2.60%,33.08%,30.66%,20.99%,2.67%.2.The disease duration(Spearman=0.1548),lesion area(Spearman=0.1973) and treatment time(Spearman=0.1021) were independently related to the DLQI. Gender was significantly associated with the DLQI score,women reached a higher overall score(P=0.0035).Examined across different marriage status groups,the total DLQI score for vitiligo changed a lot(P=0.003,married>single).During the disease type groups,universal type got the highest DLQI score,while localized one was on the contrary.We observed itching feeling,white hair and cost effect on DLQI score.3.The level of ET-1 in the plasma of patients with vitiligo was significantly higher than the normal controls(P=0.0000).We found no interaction of disease type (universal and non-universal,P=0.0707;segmental and non-segmental,P=0.1908) and disease stage(P=0.4046) on the level of ET-1 in the plasma.And the level of ET-1 in the skin tissue fluid did not differ significantly between vitiliginous lesion and the normal skin(P=0.5880).Conclusion:1.Vitiligo had moderate effect on patient's life.The disease duration,lesion area and treatment time were independently related to the DLQI.A significant association between gender or marriage status and DLQI score.We observed disease type,itching feeling,white hair and cost had great effect on DLQI score.2.The level of ET-1 in the plasma of patients with vitiligo was significantly higher than the normal controls.And the level of ET-1 in the skin tissue fluid did not differ significantly between vitiliginous lesion and the normal skin.
