A Valuation Of Confocal Laser Endomicroscopy In The Diagnosis Of Colorectal Diseases

[Background and Aims]This study focuses on the diagnostic value of confocal laser endomicroscopy(CLE) in large intestine diseases,such as tubular adenoma,adenoma polypi and chronic colitis.Histology diagnostic and grading criteria was first made according to the CLE image features and then applied to suspected cases.By comparing the diagnosis to that with pathologic golden rule, its efficiency and accuracy are valued respectively..[Materials and Methods]The CLE examinations were performed with Pentax EC3870KCLE from Japan.We scanned 10 healthy people's large intestines for the study of histological characteristics of normal mucous under CLE.In Phaseâ… ,32 cases(of which,12 have colorectal tubular adenoma,12 have adenoma polypi and 8 have chronic inflammation)with definite diagnosis were examined with CLE and a diagnostic criteria based on the analysis of the features of image under microscope were set up accordingly.In Phaseâ…¡,totally 42 suspected cases, including 17 cases with tubular adenoma,15 with adenoma polypi and 10 with chronic inflammation,were diagnosed with CLE by the new criteria.In Phaseâ…¢, each CLE diagnosis result was compared with pathology diagnosis,which was accepted as a standard for valuation.[Results]The sensitivity,specificity and accuracy of CLE method in the diagnosis of colorectal tubular adenoma is100%,82.6%and with colorectal adenoma polypi,it's 100%,93.3%.While in the diagnosis of chronic colitis,the sensitivity,specificity and accuracy is 96.4%,100%and 96.2%respectively[Conclusion]CLE is a non-invasive method that can provide histology diagnosis with compatible sensitivity,specificity and accuracy and it can be a valuable alternative in the diagnosis of tubular adenoma,adenoma polypi and chronic inflammation..
