| ObjectiveClinical part:Through prospective clinical randomized control study,we observed the clinical effect of Qingfei quyu decoction(decoction for clearing lung fire and eliminating blood stasis) for the infantile respiratory syncytial virus(RSV) pneumonia;Experimental part:Through experimental research on the inhibition of the proliferation of A549 cells by Qingfei quyu decoction,lab evidence was provided for clinically generalized application.MethodClinical part:Prospective clinical randomized control study was made through DME method of clinical epidemiology.60 cases of infantile patients with R.S.virus pneumonia who accord with the diagnostic criteria,and whose TCM syndrome type belongs to lung blockage due to wind-heat or lung blockage due to phlegm-heat were collected and randomized.The observing group was treated by basic western medicine together with Qingfei quyu decoction p.o., while the control group was treated by mere western medicine;the total course of treatment were 10 days.The general curative effect,the extinction time of fever,cough and pulmonary positive physical signs along with the thoracic X-ray photograph were compared between groups.Experimental part:the in vitro experiment of Qingfei quyu decoction' s anti-RSV effect was conducted.We observed the cytotoxicity of Qingfei quyu decoction on A549 cells by using MTT method;after that,observed the protective effect for the cytopathic effect of A549 cells by Qingfei quyu decoction below median toxic concentration to learn the in vitro antivirus effect of the drug.Result:This research shows that the cure rate of the observing group is 80.0%, while that of the control group is 60.0%,in comparison of the two groups, the difference has statistical significance(P<0.05),i.e.,the cure rate of the observing group is apparently higher than that of the control group. But for the general healing effect,there is no statistical significant difference between the two groups.There is no statistical significance in the cease time of fever between observing and control group(P>0.05),and we think it is because that there is self-limitation in the fever and the time length is short.The extinction time of cough,rales of lung in observing group is significantly shorter than that of the control group,and it is statistically significant on the difference between groups.After the course of treatment,there are 23 cases whose X-ray lung examination had recovered to normal,accounting for 76.7%;while in the control group,there are only 10 cases,which shows statistical significance in the difference.We also conducted in vitro anti-RSV experiment of Qingfei quyu decoction, showing that below the maximum nontoxic concentration 62.5 mg/ml it can completely inhibit RS virus,in the concentration of 31.25mg/ml it can suppress the cytopathic effect of A549 cells caused by 50%viruses,which imply that this formula has good in vitro anti-RSV effect.ConclutionFrom clinical research we know that Qingfei quyu decoction has obvious curative effect on infantile respiratory syncytial virus pneumonia,which can accelerate the absorption of pulmonary inflammation,enhance the healing rate of pneumonia and apparently shorten the course of disease.From experimental research we know that the cell model of A549 cells infected by RSV are feasible;the Qingfei quyu decoction has good in vitro anti-RSV effect.
