Systematic Review Of Clinical And Pathological Characters Of PBC-AIH Overlap Syndrome In Chinese

Objective To study the clinical and pathological features of primary biliary cirrhosis-autoimmune hepatitis(PBC-AIH) overlap syndrome,to elevate the recognition to the disease.Methods We retrieval all the Chinese literatures which were enunciabled publicly during 1980 and 2008 with docuterms such as primary biliary cirrhosis,autoimmune hepatitis,overlap syndrome,and analyze them.Result(1) PBC-AIH overlap syndrome is more common in middle-aged women.The male-female ratio is 1:7.3,age of onset is 47.4±10.8 years old.The mean course of disease is 35.4 months.(2) The main clinical symptoms and signs are debilitation,poor appetite,jaundice,itch of skin,intumesce of liver and spleen. (3) Alanine transaminase(ALT),aspartate transaminase(AST),alkaline phosphatase(ALP),glutamyl transpeptidase(GGT),immunoglobulinG(IgG),immunogolobulinM(IgM) is obviously higher than normal.(4) The positive ratio of antinuclear antibody(ANA) and anti-mitochondrial antibody(AMA) is high.(5) It has the histopathological characteristics of both PBC and AIH.Conclusion(1)The age of onset,gender,course of disease and clinical presentation of PBC-AIH overlap syndrome have no significant difference compared with PBC and AIH.(2)The level of ALT and AST are higher than that of PBC,and the level of ALP and GGT are higher than that of AIH.(3) Positive AMA/AMA-M2 and high title of ANA are the immune characters of PBC-AIH overlap syndrome.(4) Medium and severe interface hepatitis or patch necrosis are important in the histodiagnosis of PBC combining with AIH,and affections of bile duct are important in the histodiagnosis of AIH combining with PBC.
