Object:To investigate the relationship of abdominal obesity and coronary heart disease(CHD) by analyzing waist-to-height ratio(WHtR),the risk factors of coronary heart disease and the conclusion of coronary angiography.Methods:One hundred and fifty patients(male 114 and female 36) aged from thirty to seventy-eight(mean age 57.13±10.62 years) in the hospital with suspected coronary heart disease during August 2008 to February 2009 were studies by the case-control method.All patients' total cholesterol(TC),total triglyceride(TG),high density lipoprotein-cholesterol(HDL-C),low density lipoprotein-cholesterol(LDL-C),serum uric acid(UA),fasting plasma glucose(FBG) and blood pressure were detected by the standard methods in 3 days after hospitalization.We calculated the ratio of LDL-C/HDL-C and TG/HDL-C.Body surveying index consisting of the patients' height ,weight and waist circumference was measured,waist-to--height ratio(WHtR) and body mass index(BMI) were computed.According to the WHtR cut-off point,we enroll men whose WHtR>0.5 as abnormal WHtR group(n=102),the others as normal WHtR group(n=48).Selected coronary angiography was completed by special cardiac catheterization doctor to gain various vascular picture of the heart.All patients were also divided into groups according to the results of coronary angiography:CHD group(n=100),non-CHD group(n=50).Date were pooled to analyze the level of blood lipids,UA,FBG,blood pressure in abdominal obesity group versus(VS) non--abdominal obesity group.Multiple linear correlation analysis were applied to assess the correlationship between WHtR,BMI and the aforementioned biochemical indicators,number of coronary involved vessels.CAD risk factors were also collected for logistic multivariate regression analysis.Results:1.Compared with normal WHtR group,TG level,TG/HDL-C ratio and LDL-C/HDL-C ratio were significantly elevated(P<0.01),TC level,LDL-C level,SBP and DBP were heightened (P<0.05),but HDL-C level was significantly lowered in the abnormal WHtR group(P<0.01).Patients with abnormal WHtR had higher incidence of coronary heart disease(CHD)(84 vs.16,P = 0.000) than those with normal WHtR.2.Multiple linear correlation analysis showed WHtR was positively correlated with TG, HDL-C level,LDL-C/HDL-C ratio and number of coronary involved vessels(P<0.05),While BMIdidn't agree with the same results with those factors like WHtR.WHtR compared to BMI correlate to TC,LDL-C,FBG,UA level,TG/HDL-C ratio and SBP closely(P<0.05~0.01).3.The impact of coronary heart disease occurred in single-factor analysis showed:gender, WHtR,TC,TG,HDL-C,LDL-C,LDL-C/HDL-C,TG / HDL-C,UA,SBP,DBP and the incidence of coronary heart disease the existence of significant correlation(P<0.01),CHDgroup WHtR,TC,TG,LDL-C,TG / HDL-C and LDL-C/HDL-C,UA,SBP and DBP were significantly higher than that of non-CHD group(P<0.01),and HDL-C decreased significantly(P<0.01).4.Logistic multivariate analysis indicated that gender[beta(β)=-1.706,P=-0.007,odds ratio (OR)=0.182],WHtR[β=12.386,P=0.019,OR=23.930],LDL-C/HDL-C ratio[β=2.424, P=0.000,OR = 11.290]were closely related to the hazard of CHD.Conclusion:WHtR is a simple thermometer surveying index assessing abdominal obesity and with a higher WHtR indicated various risk factors of CHD accumulating.WHtR with others simple biochemical indicators can arrive at a preliminary diagnosis to CHD.Health status could obviously improve if we could control WHtR under normal line in effect.
