Objective:To investigate the effectiveness of preoperative biliary drainage(PBD) in patients with malignancy obstructive jaundice resulting from tumors. Methods:To retrospective analyze 63 patients undergoing malignancy obstructive jaundice in our hospital.Comparison was made of PBD versus no PBD in malignancy jaundiced patients undergoing resection of a tumor.Outcome measures were perioperative situation,in-hospital death rate, overall complications resulting from the treatment modality. Results:The level of TB in the PBD group at the beginng of they be in hospital was (268±70)μmol/L,average biliary drainage day was 14.2D.Before they had operation the level of TB down to(174±55 )μmo 1/L.Compared with(268±70)μmol/L the difference was markedness(t=2.16,P<0.05).The level of TB in the no PBD group was(148±85)μmol/L before the operation.Compared with PBD group in the corresponding period the difference was markedness(t=2.25,P<0.05).The PBD group used RBC(286±91)ml,and the other group used RBC(390±85)ml.Compared with them the difference was markedness(P<0.05).The overall complication rate was 39.7 %(25/63).There showed no difference in the overall complication rate between patients who had PBD47.2%(17/36) and those who had surgery without PBD37.0% (10/27).The single complication rate between the two group also had no difference. Conclusions:This analysis shows that PBD with current standards for patients with obstructive jaundice resulting can reduce TB level. The PBD group and no PBD group had no difference in the complication rate and death rate. It's possibly just because of PBD that made they have no difference. If the PBD group operated when TB level was (268±70)μmol/L the complication rate and death rate must higher.
