The Method And Experience Of Treating The Neck In Wide-necked Aneurysm Embolization

Objectives:Summarize the method and expericece of balloon or stent assist technique in wide-necked aneurysm embolization,and evaluate the two techniques.Methods:Retrospectively analyse the information of 48 patients with 51 wide-necked aneurysms.24 patients with 25 wide-necked aneurysms were treated by balloon(Hyperform) assist coiling,and another 24 patients with 26 wide-necked aneurysms were treated by stent(Neuroform,LEO,balloon- expending stent) assist coiling .summarize the technique and effect of the two methods and evaluate the value and problems of them .Result:The immediate angiographic result after occlusion shows that in the 24 patients with 25wide-necked aneurysms treated by balloon assist coiling,18(72%)aneurysms were completely occlusion and 7(28%)neck remnants by using 24 Hyperform balloons.In one patient the coil protrusion into the parent artery during occlusion,in another patient,the aneurysm ruptured during occlusion,and vessel spasm were observeed in two patients.22 Neuroform stents,2 LEO stents and 1 balloon-expanding stent were deployed successfully in 24 patients with 26 aneurysms that were treated by stent assist coiling.In one patient with two aneurysms in basilar artery,only one stent was deployed which covered the two aneurysms fully. 16(62%) aneurysms were completely occlusion , 8(30%)subcompletely occlusion and 1(4%)neck remnants. Without coiling ,1(4%)patient was deployed a stent only and vessel spasm occurred in one patient.Conclusion:These result indicate that balloon or stent assist coil embolization are safe and effective techniques in treating wide-necked aneurysms.The two techniques both have advantage and weakness,They have their own indication.
