Objective:To evaluate the short-term efficacy of Percutaneous transluminal septal myocardial ablation(PTSMA) on hypertrophic obstructive cardiomyopathy(HOCM).Methods:From november 2005 to february 2009,28 patients with symptomatic HOCM underwent PTSMA by Sigwart method.The pressure gradient of left ventricular out floow tract(LVOTPG),thickness of septal myocardium(IVS),width of left ventricular out floow tract(LVOT),ejection fraction(EF) and electrocardiograms(ECG) were measured before and 1 month after PTSMA.Results:PTSMA was performed 28 patients,the average alcohol injected into Septal branch was 2.46 mL;the average LVOTPG before operation was 67.7±33.6 mmHg,and after operation it reduced to 29.5±25.6 mmHg(P<0.01) and IVS was reduced 2.5±3.5 mm;2 or more than 2 septal branch were ablated in 13 patients;5 patients had transitoryⅢ°atrio-ventricular block(AVB) during operation;complete right branch block(CRBB) was found in 9 patients,all the clinical symptoms were relieved more or less.Conclusions:Short-term follow up data showed that PTSMA could significanty reduce the thickness of septal moyocardium and expand the width of LVOT and improve the symptom of patients with HOCM.In order to reduce severe complications,indications must be controlled strictly.
