Objectives:To evaluate the security of simultaneous versus staged resection of synchronous liver metastases from colorectal cancer.Methods:Pubmed/Medline,ISI Web of Knowledge,Springer link,ebscohost,Elsevier Wiley Interscience,Google Scholar were searched for case-control studies concerning simultaneous versus staged resection of synchronous liver metastases from colorectal cancer between 1989 and March 2009,Meta-analysis's method was performed to analyse morbidity and perioperative period mortality.Results:7 case-control studies,with total number of 1390 patients with liver metastases from colorectal cancer undergoing curative hepatic resection were reviewed, of which simultaneous and staged resection was 495 and 895,respectively.Comparing to staged group,significant differences were found in simultaneous group in terms of perioperative period mortality【Peto OR 3.39,95%CI(1.29,8.93),P=0.01】,and no significant differences were found in simultaneous group in terms of morbidity【OR(random) 0.88,95%CI(0.51,1.51),P=0.64】.Staged resection was to some extent lower than simultaneous resection in terms of perioperative period mortality. Conclusions:selective staged resection was safe and reasonable for synchronous liver metastases from colorectal cancer.
