Background:Mild cognitive impairment(MCI) is a syndrome characterized by subjective and objective cognitive impairment in the absence of dementia.1 MCI subclassifications include an amnestic form characterized by isolated memory impairments and one with multiple cognitive deficits,and nonamnestic form characterized by single domain or one with multiple domain cognitive impairment without memory.And the elder with subjective cognitive complaints especially memory complaints may perform normally tested by neuropsychological battery.Why it happens and what relationship between MCI and SMC is paid more and more attention.2 Although it is documented that persons with MCI have an increased risk of Alzheimer disease(AD),population- based studies have shown that only one-third develop dementia,whereas others improve,reverting to a normal level of cognitive functioning,and some die,suggesting multiple etiologies for MCI.Memory loss as the predominant complaint strongly predicts progression to dementia due to probable Alzheimer's disease(AD) in those individuals the term amnestic MCI(aMCI) is used.Objective:To examine the possible role of neuropsychologycal disturbances in MCI,we investigated neuropsychologycal setting in persons with MCI in a longitudinal study.Specific aims include 1) investigating the Conversion rate of subtypes of aMCI to AD;2) assessing the contribution of neuropsychologycal disturbance in progression from MCI to AD over 2 years;and 3) examining whether any subtype of neuropsychologycal tests predict MCI in persons with SMCs.Method and materials:Subjects from Memory Clinic of Huashan hospital including one hundred forteen of who met the operational criteria for Mayo Clinic defined MCI and eighty three of who definited as SMC without objective cognitive impairment tested by neuropsychologycal battary.They underwent multidimensional assessment and a neuropsychological battery at baseline and at follow-up,after about 2 years.Diagnosis for dementia was based on DSM-IV criteria.Result:47 cases of 140 elder people with MCI convert to AD with 44 of amnestic MCI subjects including 17 aMCI-s and 27 aMCI-m subjects of 65 progressed to probable AD within an average 23.80(6.78) months[mean age 69.59 (8.22) years,MMSE 25.50(1.50)).24 cases of 140 returned to SMC whitout objective cognitive deficits.There is no significantlly difference between the reversion rate of aMCI-s and aMCI-m(77.3%vs 85.9%,p=0.203,>0.05).For the eighty three SMC,forty eight cases(57.8%) remain SMC and twenty six cases(31.3%) progressed to aMCI,and other night cases(10.8%)to naMCI.SMC subjects progressed to MCI were much older and with poor visual space performance at baseline.Conclusion:The aMCI-m more likely to progress to AD than aMCI-s.The SMC may convert to MCI and the converted were older and performed poor in visual space skill test.
